 SUMMARY:

  The Institute for Family Health (Institute) is a non-profit organization committed to providing health

  services to medically underserved communities. The Institute operates

26 full and part time health

  centers throughout the Bronx, Manhattan and the mid-Hudson Valley region of New York State.



  The Program Assistant role will include support of Institute training activities, primarily through the

  Health Workforce Retraining Initiative (HWRI) grant, and general support of the Planning and

  Development Department staff. HWRI is a New York State Department of Health grant-funded program

  aimed at training health center staff, both at the Institute and at partner organizations, in team-based

  care, health information technology and care coordination. The Planning and Development Department

  functions include: external communications, project management, grant writing and student placement.



 Responsibilities

 · Assist in the preparation and submission of program reports

  · Work directly with staff throughout the Institute to gather training data  · Schedule trainings, coordinate trainers and materials as needed  · Track and report training participation  · Assist with tracking and submitting expenses for reimbursement  · Organize meetings

 Community Health Worker, Northern Queens Health Insurance Enrollment



  Public Health Solutions’ Access to Health and Food Benefits program has been funded for two years by the Center for Medicare and Medicaid Services (CMS) Child Health Insurance Program Reauthorization Act

(CHIPRA) to help enroll eligible Hispanic children and families in Northern Queens in public insurance programs.  The Access to Health and Food Benefits program at Public Health Solutions has historically helped New York City residents obtain free or low-cost health insurance coverage by providing one-on-one assistance with the application and the renewal process.  This new project will employ Community Health Workers to actively engage those in the Northern Queens community and provide comprehensive information about public health insurance options and their related benefits.  The Community Health worker will educate his/her peers and community members about the importance of health insurance in general and how families can apply for public health insurance for themselves and their children.



  Specifically, the Community Health Worker, Northern Queens Enrollment

will:

  ·         Reach out to the community in Northern Queens to educate them  

about Health insurance availability.

  ·         Conduct scheduled workshops and information sessions  

regarding public health insurance eligibility, documentation requirements, and benefits at community organizations that have ongoing relationships with potentially eligible clients.

  ·         Spread accurate and culturally specific information and  

messages about public health insurance.

  ·         Work with community members and attend community events  

around Northern Queens to dispel myths about public health insurance eligibility and enrollment locations.

  ·         Distribute outreach materials to community-based  

organizations, doctor’s offices, and other key community hubs through mailings and in-person.

  ·         Build calendar of outreach activities and events to expand  

reach within the community.

  ·         Assist in development of tailored outreach/education messages  

for Northern Queens community.

 ·         Support effort to establish social media presence.



 Qualifications:

  ·         A minimum of 1 years’ experience working in a community-based  

setting

  ·         Working knowledge of community health, advocacy  

communication, and community organizing

 ·         Familiarity with communities in Queens, NY

  ·         Excellent oral and written communication skills, along with  

knowledge of database and word processing systems

  ·         Must be a creative problem solver and be comfortable working  

in a multi-cultural setting.

 Job description



 Healthy Checkout Line Evaluation Intern





 Background

  In Spring/Summer of 2014, the New York City DOHMH will be launching an evaluation of the installation of “Healthy Checkout Lines” in select grocery stores in the Bronx.  The Healthy Checkout Line (HCL) is a new element of the NYC Health Department’s Shop Healthy program.



  For this evaluation, participating grocery stores will feature an HCL that will contain only healthier food options or non-food options.  HCL signage will also be placed throughout the store and at the checkout line.  This evaluation seeks to determine the effectiveness of HCLs as compared with traditional checkout lines.



  The evaluation will involve testing the feasibility of installing the checkout lines, data collection to understand customer flow at stores, and surveys with approximately 1,000 customers.



  Roles and responsibilities The DOHMH Shop Healthy NYC! program is looking for interns to assist with implementation of the HCL evaluation.  

  The internship will take place from June 2, 2014 ending on June 15 2014, requiring a minimum of 15-21 hours/week.  This person will be responsible for survey administration, maintenance of healthy checkout line displays, data entry, and other tasks as needed.

  The Evaluation Coordinator/Survey Administrator role is a part-time job (&lt; 20 hours a week) which will assist

  in data collection in the field, coordinating and scheduling data collection volunteers and helping with data  collection related to City Harvest Healthy Neighborhood programming.



 Key Responsibilities:

  Coordinate all field work related to evaluation & monitoring, ensuring that data collection shifts are populated

  in accordance with program calendar/implementation and evaluation study plans; coordinates volunteers, and

  maintains department calendar and data entry and cleaning protocols.  

Specific activities will include but not  limited to:



  · Scheduling and maintaining an active roster of all data collection activities and ensures coverage for  all field work, including survey data collection/interviews.

  · Working with Volunteer Services Department and community partners to routinely identify, match  and place volunteers/survey administrators with evaluation projects

  · Ensuring that administration of evaluation tools and other data collection projects are in line with the

  evaluation plan designed for such study, identifies barriers to achieving plan objectives and promptly  notifies the Director of barriers

  · Building and cultivating partnerships/relationships with community groups and institutions toward

  identifying a cadre of community volunteers to support evaluation goals, including data collection

  · Maintaining Evaluation Department calendar, working with staff members to track project milestones

  and works with staff to resolve any challenges to completing required projects

  · Managing all follow-up survey, focus groups and other qualitative data collection activities.

  · Conducing and coordinating data entry and quality control processes, ensuring data is entered in a  timely manner and ready for data analysis and reporting

  · Developing and maintaining databases for qualitative data entry and analysis.

  · Coordinate day to day activities related to different projects, providing general administrative support

  as required to meet the department’s objectives; provides support in standardize presentation slides,  reports and other documents and correspondence.

  · Routinely searches scientific databases, online resources to identify and summarize research findings,

  identify emerging trends in relation to emergency food, hunger, nutrition and chronic diseases, and  related research  · Provide general administrative support to the department.
