Nutritionist – “Just Say Yes to Fruits and Vegetables” (JSY) Program



GENERAL STATEMENT

The “Just Say Yes to Fruits and Vegetables” Nutritionist is a grant-funded position by the New York State Department of Health, Hunger Prevention Nutrition Assistance Program and USDA. Responsible for planning and implementing Nutrition Education to The Food Bank for Westchester member programs with a major focus on SNAP eligible clients. This position may require some "after business' and week end hours. This position reports to the Director of Member Services and Nutrition.

QUALIFICATIONS

• Registered Dietitian preferred OR

• BS in Nutrition OR

• BS in Public Health with a minimum 15 credits in nutrition

• Must have 2 years community nutrition/public health experience

• Strong computer skills

• Travel required with possible overnight travel

• Serv-safe certified within first 6 months of employment.

RESPONSIBILITIES

This position reports directly to the Director of Member Services and will work in tandem with the Agency Relations Manager and the Food Growing Project Coordinator.

Specific duties will include, but not limited to:

1. JSY Nutritionist/Consultant will perform the following services for the Food Bank in the capacity of a Public Health Nutritionist:

• Assist in the implementation of a comprehensive nutrition education and obesity prevention program for low-income individuals accessing emergency food programs.



• Identify food pantries, shelters and soup kitchens who will participate in the Supplemental Nutrition Assistance Education Program (SNAP-Ed).



• Schedule and conduct nutrition education sessions and recipe demonstrations with SNAP- eligible participants.



• Market JSY nutrition education sessions to food pantry operators and community partners to highlight nutrition education training opportunities that are available to food stamp eligible populations.



• Participate in activities sponsored by the Food Banks, HPNAP, community groups and other partners to facilitate SNAP-Ed program objectives.



• Conduct a minimum of 10 nutrition education sessions with at least 100 SNAP eligible participants per month using lesson plan materials provided.



• Utilize learner-centered and facilitated group discussion teaching techniques to enhance effectiveness of nutrition education sessions.



• Maintain regional calendar / contact information for JSY website.



• Identify community events that support SNAP-Ed goals for the intended target audience.



• Provide written reports to NYSDOH.



• Actively participate in all JSY team conference calls and events. Strive to manage all resources wisely for the maximum use of the Program.



• Seek opportunities for professional development to improve effectiveness in the performance of job duties.



• Participate in other SNAP-Ed related duties as “assigned” (ex. curriculum redesign, recipe development, etc).



2. Program Activities

• Work closely with appropriate Food Bank staff and member agency staff to conduct workshops at Direct Service locations, such as Kraft Mobile Food Pantry locations.



• Work in collaboration with the Food Bank staff and Department of Health to assess the emergency food programs prior to developing policies and implementing environmental changes to support health promotion and obesity prevention activities.



• Assist Agency Relations Coordinator to develop programs and training for all members relevant to marketing JSY.



3. Other:

• Contribute to the monthly newsletter, nutrition web page and other publications as needed.



• Market food products to member programs and their clients, as needed.



• Other duties as assigned.



PHYSICAL REQUIREMENTS AND WORKING CONDITIONS:

Ability to enter and access information on computers and desk top publishing systems.

Ability to file and retrieve information from office flies.

Ability to access all work sites of the agency.

Ability to travel independently in the day time and evening.

Ability to give speeches and presentations to groups.

Ability to lift, carry, and set - up a variety of promotional materials including VCRs, materials in boxes, and table top screens (weighing up to 25 pounds).

Ability to lift 25 pounds.

Ability to pack and unpack and put away various materials.



WORKING CONDITIONS:

Working inside in office environment.

Working outside in a variety of conditions on special events/promotional projects (targeting SNAP –Ed audiences).

Travel throughout The Food Bank for Westchester service area and NYS on occasion.

SUMMARY: Provides program and administrative support to the Department of Family Medicine and Community Health at the Icahn School of Medicine at Mount Sinai operated by the Institute for Family Health.



The Institute for Family Health (IFH) is a non-profit organization committed to providing comprehensive primary health services to disadvantaged communities. In addition to operating 28 health centers in the Bronx, Manhattan and upstate New York, the Institute leads and operates the Department of Family Medicine and Community Health at Mount Sinai Medical Center. The Department is responsible for teaching and mentoring in family medicine within the medical school, overseeing the Institute’s three family medicine residency programs, and conducting research.



We are looking for an energetic staff member with good attention to detail to assist in our growing department. This is a great opportunity for a recent graduate or part-time student to gain experience in the fields of health and non-profit administration and management. The department coordinator will work with a team of staff, including the Department Chair, the Department Administrator, and the Director of Inpatient Services. This is a full-time position located at the Icahn School of Medicine at Mount Sinai.



The Department Coordinator will work independently on a variety of activities, including:



• Serve as a liaison with other Mount Sinai departments and services;

• Manage departmental faculty appointments and hospital privileges;

• Coordinate events, include Grand Rounds lectures, special events, and related medical student activities;

• Assist with departmental communications, including maintenance of the department website;

• Schedule and prepare documents for meetings;

• Provide general office support and administrative support for the inpatient service.

Qualifications:

1. Master’s degree in Public Health and minimum of 2‐3 years of related job experience internationally

2. Strong interest in reproductive health technologies; knowledge of issues related to PPH care highly desirable

3. One or more years of statistical training in quantitative methods and solid understanding of statistical software

packages, such as SPSS

4. Willing to travel internationally 20‐30%

5. Working knowledge of French or Spanish

6. Some experience with clinical trials, human subjects research and regulatory agencies highly desirable

Manager, Engagement and Advocacy

JOB DESCRIPTION

The Registered Dietician (RD) reports to the Nutrition Team Manager to conduct nutritional assessments and determine dietary needs for a frail adult population to achieve an optimal level of independent functioning. The RD provides necessary consultation and teaching to members, families/caregivers, and the care teams of Senior Health Partners (SHP) teams and Healthfirst Complete Care, and other integrated products, to ensure the mission of the program is maintained.

The RD frequently communicates complex information and interacts through electronic documentation/tools. This is a paperless work environment requiring daily hands-on administration of multiple electronic Patient Health Information (PHI) databases and security requirement tools such as encryption. These systems include, but are not limited to CCMS (Care Enhance Clinical Management Software), Sun guard Maces Service Module (electronic archiving), Right Fax, VoIP, Virtual Work Platforms (VPN), scanning and creating PDF files (Adobe Acrobat), and MS Office 2010 software (such as Word and Outlook).

• Deliver and direct nutritional care and services for members in collaboration with Care Management Team (CMT) offering nutritional counseling and education through case findings and nutrition screening.

• Evaluate members’ nutritional status utilizing various assessment tools and obtains primary care physician’s orders as required. Initial evaluation, reassessments and/or referrals from CMTs will be completed by phone or through a home visit.

• Calculate nutrient intake integrating the nutrition treatment plan into the overall care plan; collect laboratory data, and consult with the Care Management Team, Nutrition Team Manager, Clinical Manager and Medical Director as appropriate; assess food preferences, preferred methods of preparation including cultural and environmental factors influencing dietary intake.

• Educate members and care givers about the need for and preparation of therapeutic diets as part of the care plan.

• Collaborate with Care Management Teams through daily communication and team meetings to develop an optimal plan of care for member(s).

• Monitor and sign-off assessments from Diet Technician(s)/Diet Technician Registered(s) ensuring an appropriate care plan, and pertinent documentations are completed in an accurate and timely manner. Assist with providing preceptor and training accordingly.

• Work with Care Management Teams to support the company''s mission and program goals by providing pertinent information and coordinate between dietary/other services, participating in care planning meetings including formulating and implementing policies/procedures. Participate in quality management activities and educational programs designed for members, families/caregivers and staff.

Minimum Qualifications:

• Bachelor''s degree and Registered Dietitian in good standing.

• One year of related industry experience.

• Advanced clinical and home health care knowledge.

• Technical experience managing patient health information (PHI) in a database module and archival system (e.g. Macess).

• Thorough knowledge of current community health practices for adults.

• Working knowledge of the interdisciplinary model of care management.

• Proficiency in navigating internet browsers, computer hardware and multi-tasking with multiple electronic documentation systems (toggling).

• Strong proficiency in Microsoft Word (documenting, writing procedures, recommendations, etc); Excel (sorting, tracking, formulas, pivot tables, etc); time tracking systems; familiar with Corporate email/calendar system; managing electronic patient health information (PHI).

• Meet Requirements for State registration with the Uniform Assessment System (UAS): Unexpired NYS driver’s license or NYS non-driver’s license ID (DMV Identification). If residing outside of NYS, candidate must obtain a NYS non-driver’s ID before commencement of employment. State registration with UAS for long-term care in NY State is required to access for training, testing and system usage. Upon completion of UAS training, the State will administer testing online. If a passing score is not achieved, the State will allow the individual to retake the test once. The UAS access is a necessity for this position, so employment is contingent on acceptance and continued authorization by the State to have UAS access.

Telecommuter requirements:

Ability to work remotely is dependent upon completion of training and passing audit scores, which varies by department. It is also contingent on the following telecommuter requirements:

• Designated work area at the Telecommuting location away from the main flow of home traffic which meets OSHA and departmental policy.

• Broadband solution will be a cable modem type using an Ethernet connection from a reputable provider such as Comcast, Cox Cable or Cablevision, TW or fiber service such as FIOS or AT&T Uverse offered by Verizon, AT&T or regional partners.

• Usage of a DSL broadband is not recommended and will not be supported by Healthfirst’s Technical Services team, additionally; wireless is also not recommended with both options requiring prior approval of the division Vice President and Human Resources.
