The Division of Health Interview Statistics has just released a draft of the 2015 NHIS Survey Questionnaire. In addition to the NHIS core questionnaires, there are supplemental questions on family disability as well as sample adult functioning and disability. In addition, there are supplemental questions on occupational health, epilepsy, hepatitis b and c screening, food security, enhanced questions on health care access and utilization, immunization, ulcerative colitis and Crohn’s disease, and a cancer control module which includes questions on diet and nutrition, physical activity, tobacco use, screening exams, genetic counseling/testing and family history of cancer.
