Dear Prospective Post-Bac students:

Below you will find the link to the Mid-Semester Evaluation Form; one form must be submitted for EACH course you are currently taking. You will need your instructor's e-mail address to complete the form.

Once you submit this form, a request will be forwarded to instructor via e-mail. Please let them know that you are submitting this request, so they know what to expect.

You will receive confirmation via e-mail as soon as the instructors have submitted the evaluation; Please DO NOT e-mail us to request confirmation. 

Please enter all information carefully, as any incorrect information especially e-mail addresses will delay your application.

Remember all requests must be made by October 15th, 2010. The form will be disabled after that date.
