"Brooklyn Stay Well, Enjoy Life" 

Medical Case Manager

     

           Since 1994, The Family Center has provided integrated family
services to families in Central Brooklyn. Our newest program, Brooklyn Stay
Well, Enjoy Life ("B-SWEL"), is a community health and wellness program to
address diabetes in minority communities, coordinated from our Bedford
Stuyvesant office. As a collaborative initiative with two other community
providers, this three-year program will provide 1,200 Central Brooklyn
residents with screening, education, and diabetes prevention activities. It
includes a six-month wellness and lifestyle intervention stressing diet,
exercise, and social support. 

 

The Medical Case Manager ensures that clients are linked to appropriate
primary medical care for pre-diabetes, provided with up-to-date, clear
information about pre-diabetes, diabetes, and weight management, coordinates
treatment, care, and information from partner providers, and provides
education and support for clients' positive health care decisions regarding
weight and diabetes.  The Medical Case Manager reports to the B-SWEL Project
Director.

 

RESPONSIBILITIES

.          Ensure that clients are linked with and followed by an
appropriate primary care provider; 

.          Confirm that quality of medical care meets public health service
guidelines; 

.          Work with staff to ensure that health care follow up protocols
are completed on a timely basis;

.          Provide home- and office-based diabetes education and lifestyle
counseling to clients;

.          Conduct groups with a focus on educating and supporting community
members with lifestyle changes to lower diabetes risk;

.          Obtain and record lab reports from clients' primary health care
providers; 

.          Ensure that all client medical information is documented in
charts and data system;

.          Monitor quality of client health care information and complete
documentation as needed;

.          Special projects as required by supervisor;

 

MINIMUM REQUIREMENTS

.          BA, preferred in nutrition/dietetics, nursing, health education,
public health or related field with at least 2 years experience

.          Experience conducting health-related trainings, group
facilitation as well as individual counseling

.          Knowledge and familiarity with local community and cultural
resources 

.          Experience in diabetes, weight management, or nutrition support 

.          Bilingual, Spanish or Mandarin/Cantonese language 

.          Schedule flexibility permitting occasional evening and weekend
work 

.          Ability to conduct home visits as required
