Position Summary:
NYSHealth is seeking an interim Program Associate to manage the Meeting the Mark: Achieving Excellence in Diabetes Care program and request for proposals (RFP). The Foundation's goal is to help at least 3,000 primary care physicians in New York State provide excellent care for their patients with diabetes, as demonstrated by achieving recognition through the National Committee for Quality Assurance (NCQA) Diabetes Recognition Program or the Bridges to Excellence (BTE) Diabetes Care Recognition Program. Salary is commensurate with experience. It is expected that the candidate can commit up to 30 hours a week. • Review letters of intent and applications for Meeting the Mark; help with due diligence process.
• Provide technical assistance to grantees regarding submission process and practice improvement strategies as needed.
• Track progress of new and existing Meeting the Mark grants.
• Learn the requirements and the submission process for the NCQA Diabetes Recognition Program.
• Post relevant resources and grantee updates to the NYSHealth and Fulldiabetescare websites.

The Program Associate reports to the Senior Program Director and will also work closely with grants management staff.

Qualifications:
This is an ideal part-time position for a Master of Public Health (or related field) candidate. The Program Associate should have the following qualities:

• Excellent interpersonal skills; previous training/teaching experience (preferred).

Montefiore Medical Center, the university hospital for the Albert Einstein College of Medicine, operates four acute care hospitals with more than 1,500 beds, including the 126-bed Children's Hospital that provides primary, specialty and subspecialty health care to infants and children up to 21 years old. The Children's Hospital at Montefiore is the principal hospital for the children of the Bronx and surrounding areas. The hospital treats more than 100,000 pediatric patients each year and serves as the hub of a network of over 25 community-based health centers offering comprehensive primary, specialty and subspecialty care.
The Montefiore School Health Program (MSHP) provides comprehensive medical and mental health services at 19 school-based health centers in the Bronx with a growing number of sites providing dental and community health services. The mission of MSHP, in collaboration with the Bronx community, is to eliminate health disparities facing children living in the Bronx by providing full access to high quality comprehensive primary and preventive health services regardless of one's ability to pay.
As a member of the clinical inter-disciplinary team, the Community Health Division (CHD) uses a public health approach to eliminate health disparities facing children living in the Bronx. Through a comprehensive strategy, Community Health Organizers (CHO) and Specialists (CHS) provide leadership and support in developing health-promoting school environments to improve the well-being of all students and members of their community. The CHD is responsible for coordinating the effective delivery of classroom-, school- and community-based health promotion and preventive initiatives in cooperation with the school, community members and partners, and government agencies. The MSHP Community Health Division seeks to hire an Assistant Director (AD) to advance its work and strengthen the infrastructure of the Division.
Position Summary:
The Assistant Director will report to and work closely with the Director of Community Health to direct and develop a team of Community Health Organizers and Specialists responsible for the large-scale health promotion projects based at multiple school sites and the surrounding communities. As a member of the Leadership Team, the Assistant Director shares the overall responsibility for all operational, technical, administrative, programmatic and personnel activities of the Division.
Requirements and Competencies:
The ideal candidate will have five-plus years of experience in public health administration and leadership, personnel management and development, strategic planning and execution, community relations, fiscal management and fund raising, informatics, and program administration, development and evaluation. Demonstration of successful program and staff management in a diverse, public health setting with subject matter expertise in at least two of MSHP's priority service areas, (reproductive and sexual health, asthma management, fitness and nutrition and health maintenance) is required. Candidates must hold a Master's Degree in Public Health, Community Health, Health Administration, Business/Public Administration or related public health discipline. The ability to speak Spanish will be well-received.
Leadership
The CHD supports a team environment and the Assistant Director must promote a mutually supportive, collaborative workforce within and across divisions and the hospital; manage change in a positive manner; and provide appropriate opportunities for career development. Outstanding communication skills are critical to this position. The AD needs to be a skilled, engaging and confident communicator who understands the concerns and priorities of diverse audience and can speak to each of them respectfully, insightfully and effectively.
Program design and development
The new AD will be expected to support a strong staff development plan grounded in outcome based measures of effectiveness, adept at establishing aggressive but achievable outcomes for program efforts, as well as, indicators and benchmarks for measuring progress. It is expected that the AD will be effective in helping team members appreciate the value of measurable outcomes in establishing clear direction and making strategic decisions.
Program Evaluation, Data collection and Data Analysis
The AD will support the expansion of a systematic programmatic assessment and evaluation of health promotion and prevention programs to ensure efforts are focused on implementing the most effective strategies. Experience in implementing formative and summative evaluations, impact evaluations, research design, qualitative and quantitative analysis, data collection and analysis using SAS, SPSS or other statistical software will be a plus.

DescriptionBetter Living Now (BLN), a National supplier of diabetes products has immediate opening for a Certified Diabetes Educator ( CDE) to provide diabetes education and Medical Nutrition Therapy to our patients.

In this role, the CDE will provide diabetes education and medical nutrition therapy to managed care, private pay, commercial insurance and Medicare patients by phone, in person or via alternate media (i.e. electronic, written, etc.) The CDE will also assist with development of diabetes education and general wellness education for marketing and informational materials.

Duties for this position are:
-Conducts education sessions telephonically and/or via applicable medium(s).
-Provides carbohydrate counting education to patients.
-Conducts quality monitoring.
-Duties may including assisting with the development and updating of related materials to include: Continuing Education (CE) programs, education materials, live programs, computer-based training, as well as other written materials or presentations.
- Reviews and writes articles for various BLN media and outlets.
-Coordinates education and training with professional affiliations, i.e. ADA, AADE, JDRF, etc.
-May function as an insulin pump trainer.
-Provides support and mentorship.
- Participates in local health fairs and/or travels to trade shows/programs and other events to promote/represent BLN as required.

RequirementsQualifications:
-Demonstrated competency and experience in education, professional presentations, public speaking.
-Demonstrated clinical competency in Medical Nutritional Therapy as applies to diabetes and diabetes related subject areas.
-Demonstrated clinical competency as a CDE.

Education/Experience:
Bachelor's Degree from a four-year college or university in a healthcare related field and a minimum of one year experience in diabetes patient education; or an equivalent combination of education and experience.

Certificates, Licenses, Registrations:
-Certified Diabetes Educator with current certification

AND

-Certified Insulin Pump Trainer with current certification

AND

One or both of the following:
a. Registered Dietitian credentialed by the Commission on Dietetic Registration.
b. Current state RN License.

Computer Skills:
Required: Knowledge of Microsoft Word, Excel, PowerPoint and Outlook.

I was wondering if you know of anyone that would be interested in a long-term care facility opening. The facility is called Nathaniel Witherell, located in Greenwich, CT, just a short train ride from Manhattan. The position is 32 hrs/week, which I found really helpful while still taking graduate classes. Candidates can be RD or RD-eligible. The facility is 200-beds, including long-term residents and short-term cardiac and orthopedic rehab patients. The position is primarily to cover the short-term patients, but also includes coverage of some of the long-term residents. I have been at this location for almost a year and a half now and it has been a great experience and I would highly recommend this facility and company as a great place to work!
