Job Description Title Nutritionist/Dietitian Reports To: Medical Director Program/Dept: Adult Day Health Care Center West Division: H.A.T.S.S Supervises: None Exemption Status: Exempt Creation/Revision Date: June 9, 2011 Position Description The Nutritionist/Dietitian has overall responsibility for the development and monitoring of all nutritional services of the ADHC. These services include menu preparation, consideration with food preparation staff, and the selection of client meals and snacks. This position functions in a primarily electronic environment and requires proficiency in 'Windows" based technology. Additional responsibilities include providing nutritional/therapeutic counseling and ongoing nutritional education to participants of the ADHC within specified time frames, supervising and monitoring the nutritional status of clients through written assessments, observing food intake. Essential Job Functions The following duties are mandatory requirements of the job: 1. Completes an individual client nutritional assessment within five (5) visits and in a period not to exceed thirty (30) days after admission on all clients of ADHC and every thirty (30) days thereafter. 2. Evaluates nutritional status based on developed protocols and standards, develops individualized nutritional care plans, and maintains a nutritional record in clients' chart. 3. Supervises nutritional planning, food production/preparation, dietary modifications and make all necessary adjustments on weekly menu of the ADHC with food service staff. 4. Contributes to initial and ongoing comprehensive care planning and attends comprehensive care planning meetings, morning rounds when on site. 5. Develops and facilitates group sessions in consultation with interdisciplinary team to address nutrition, menu planning, cooking, and current trends in diet therapy to clients in program. 6. Provides in-service education to the program staff and volunteers. 7. Acts as liaison to external care providers in regard to possible interaction of medical condition, drug treatment, and nutritional status. 8. Maintains client caseload for purpose of coordinating care both within program and with outside case management providers. 9. Insure the integrity and confidentiality of all patient information in the EHR by not sharing personal user name or password with other staff. 10. Insure the integrity and confidentiality of all patient information in the EHR by exiting the application or locking the system when away from the computer 11. Supports and participates in advocacy efforts to end the twin crises of homelessness and AIDS. Education and Certification Must be registered as a Dietitian from a school accredited by the American Dietetic Association. Holds Masters Degree from an accredited college or university. Two years clinical experience in a health setting preferred. Experience in working with HIV/AIDS population desirable.
