CAMBA's Food and Nutrition Services (FNS) contract provides food services that include Congregate Meals, Food Pantry Bags, Food Vouchers and nutritional services to 300 HIV positive individuals and their minor children who are residents of NYC and who meet the financial eligibility for the program. All enrolled clients will meet with our Registered Dieticians for an assessment and the completion of a Comprehensive Treatment Plan to address their food and/or nutrition needs. Clients identified as having special nutritional needs will receive appropriate services including Individual Nutritional Counseling and Group Nutritional Education. Through counseling and group education, clients will learn about meeting their own, unique nutritional needs and ensuring proper nutrition and diet while managing co-morbid conditions and treatment adherence. In counseling sessions, clients will set nutritional goals and make specific plans to achieve them. For clients whose physicians have made referrals for nutritional supplements, the appropriate supplements will be distributed in conjunction with counseling sessions. The program staff will ensure that clients are connected and remain in HIV Primary Care in order to continue receiving services under this contract.
The person filling this position is expected, under general direction, to:
(1) provide individual and/or group counseling services to assist individuals to understand the connection between proper nutrition and the relationship to a healthier lifestyle; (2) counsel clients in dealing with such issues as: behavior, educational progress, family dynamics, mental and physical health, housing, personal finances, substance abuse, mental and/or physical trauma, etc.
EDUCATION/EXPERIENCE REQUIRED:
Registered Dietician (RD), or a Certified Dietician/Nutritionist (CDN), or Registered Dietician Eligible (RDE) and experience with similar services. (As required by Funder)
OTHER REQUIREMENTS:
Must have experience working with People Living with HIV/AIDS (PLWHA).
Bilingual Spanish or Spanish / Creole
Knowledge about, understanding of, and ability to work closely with, persons with HIV/AIDS and related issues.

Nutritionist/ Registered Dietition

Job Description

ASAP
Mon - Fri 37.50 hrs

The Registered Dietitian will work as a Public Health Nutritionist (PHN) and will conduct Child and Adult Care Food Program (federally mandated) Program Reviews on approved child and adult day care centers; and review and approve Corrective Action Plan submissions.

The PHN will provide specific nutrition expertise, as it relates to infants, children, children with special health care needs, and functionally-impaired adults in a day-care setting. The PHN will function as an evaluator, contracts manager, educator, consultant, planner and coordinator.

Responsibilities:

• Work in the Metropolitan Region (5 boroughs and 5 counties).
• Monitor CACFP organization agreements and conduct annual program reviews , as mandated by the USDA and New York State Program regulations, using the CNP-Xpress (automated review system) using a laptop computer.
• Provide technical assistance to CACFP organizations as it relates to Program services, menu records, food service operations and record keeping.
• Monitor organization compliance to Program requirements such as, regulations, civil rights, tax-exempt status, Title XX Certification, Income Eligibility Applications and sanitation permits.
• Review all documents that the organization must have on file, including current CACFP Application and Management Plan, Agreement with updated Management Plan, license or other appropriate
certificate of occupancy, Letter to Household and documentation of staff training.
• Report results of Program Review and evaluation and follow-up to ensure that all corrective actions are implemented in a timely and appropriate manner.
• Submit completed review report with recommendations to PHN supervisor, in a timely manner.
• Submit written report to the organization that include steps for implementation of the corrective action.
• Respond and document request for technical assistance; during the site visit.
• Identify priorities for technical assistance based on monthly reports, and review documentation.
• Facilitate training for organizations on the menu record book and record keeping.
• Serve as a resource to the centers in activities relating to nutrition and the dietary needs of children and functionally-impaired adults in day care.
• Review reports, interpret Program data and compare statistical information.
• Must be proficient with Microsoft Office computer software programs.

Job Title: High School Coordinator

The Lang Youth Medical Program (Lang Youth) is a six-year longitudinal science enrichment, youth development program for 6th - 12th grade students who live in Washington Heights and Inwood. The goal of the program is to foster academic achievement and develop its students' habits of mind and life skills in order to position them on the path to a four-year college education and future careers in the health sciences. Lang Youth is seeking a full-time, high school coordinator for the program.
This person would be responsible for:
• Overseeing the implementation of the academic year and summer curriculum for the 10th, 11th and 12th grade Lang Youth students. The curriculum focuses on local public health, global health, and college applications and general college preparation.
• Keeping track of 10th, 11th, and 12th grade students' academic progress, coordinating academic support when needed, and following up with families and schools.
• Recruiting, interviewing, hiring and supervising academic and summer program advisors who directly deliver the Lang Youth curriculum.
• Coordinating all college preparation activities, including: two annual overnight college trips, standardized test preparation, parent and student workshops and meetings focused on the college applications process (including financial aid and scholarship applications), and one-on-one student counseling.
• Organizing internship placements and recruiting internship mentors for the summer program.
• Participating in and planning for other program-related activities including student-related celebrations, interviews for incoming students, graduation, etc.
• Maintaining program's financial database, tracking expenses, and managing accounts with outside vendors.

This position provides an opportunity to learn and expand knowledge of science enrichment, college preparatory work, and hospital operations. It requires ability to work as part of a team. The ideal candidate will possess the capacity to communicate effectively with various groups, including: young adults, parents, community members, NYP department leadership, organizations in the Hospital and Medical Center campus, and colleagues throughout the medical and academic community of Columbia University. Organizational skills and attention to detail are crucial to carrying out the responsibilities of this position successfully.

A master's level trained individual (in education, public health, or a related field) and fluency in Spanish are preferred. Three to five years of working in an educational or enrichment program setting with high school students is required. Experience working with high school students in college preparation is preferred but not required.

The individual will report to the Lang Youth Program Manager and the Director of Community Health Education, Outreach, and Marketing of the Ambulatory Care Network of the NewYork-Presbyterian Hospital.
