THE POSITION
The Nutrition Resource Manager (NRM) is responsible for comprehensive Nutrition Resources program management and quality assurance. This position develops policies, programs and educational trainings for Nutrition Resources. Coordinate and manage seed grants for urban farming and other special grant initiatives. DUTIES AND RESPONSIBILITIES:
• Ensure the use and compliance to the United Way – HPNAP nutrition standard for food purchasing.
• Promote food quality for meal service and distribution by Emergency Food Programs (EFP) through management of the food order guide and food packing guidelines.
• Coordinate Purchasing and Food Quality Projects, including analysis of agency purchasing practices; fresh produce trends, and purchasing initiatives with NYSDOH. This would include managing and developing Produce of the Month Program and other education initiatives to promote highly nutritious foods.
• Develop project management plan for Seed Grant for Urban Farming and all other assigned grants. The plan includes: application process timeline, visitation/monitoring schedule grid, progress report grid and evaluation grid; serve as "contract manager" to awardees in the execution of their projects.
• Design and develop12 monthly Nutrition and Food Safety News newsletters and special health issue supplements for member EFPs.
• Develop and conduct Food Safety workshops and education initiatives.
• Participate in quarterly state-wide HPNAP Nutrition Resources Manager meetings.
• Participate actively in state-wide and city-wide committees that propose policies for the improvement of emergency food services in NYC. These committees include:
• Food Safety & Nutrition Policy Subcommittee, a subgroup of the Policy Committee on NYC Hunger Resources and the EFP Nutrition Education Group
• Prepare and submit Quarterly Nutrition Resource Manager required and special reports for Nutrition Resources programs, projects, and staff for Contract Manager and state-wide Coordinator.
• Assist in grantee selection of Food and Operations Support funds; facilitate the selection and allocation for Seed Grant funding.
• Represent UWNYC-HPNAP at local, state, or national conferences and among community nutrition outreach efforts.

QUALIFICATIONS AND SKILLS

• Must be a Registered Dietitian with a Master's Degree in Nutrition or Public Health and with at least two years experience in Community Nutrition.

GOAL is an international humanitarian organisation founded in 1977. GOAL's programmes are aimed at assisting the most vulnerable in communities and providing immediate support to those affected by war or natural disaster. GOAL is an organisation with an approximate annual budget of €60m of which approximately 30 % is expended in health, which is a core sector for GOAL.
GOAL's health strategy centres on initiating and contributing to the sustainability of PHC activities in countries of operation in co-ordination with national governments and other relevant national and international partners. HIV and WASH are integrated in to GOALs health activities as GOAL recognises that such issues play a key role in determining population health status particularly in vulnerable communities. GOAL may consider directly implementing health activities during acute emergency interventions and in Complex Emergency (CE) situations however from the outset GOAL will consider building MoH capacity to respond and will plan an handover strategy where possible.
GOAL supports community based behaviour change approaches and works with communities often through local partners to get them actively involved and empowered to seek positive changes to their own health status and that of their community. GOALs health programme encompasses nutrition, primary health care, HIV, and WASH with a particular focus on maternal, new born and child health).
OVERVIEW OF ROLE:
To provide health technical guidance to GOAL health programmes globally with specific emphasis on a number of key countries (final focus locations to be determined). The post holder will have a particular focus on community health and health system strengthening. The Development Health Advisor will also ensure that health activities in defined countries are in line with GOALs health strategy and overall primary health care approach.
The post-holder will also work to develop key relationships with health donor NGOs such as USAID and drive networking opportunities for GOAL with health organisations such as Core and with peer agencies. The Advisor will have a particular focus on developing GOALs internal capacity on documenting the evidence of the impact of GOALs health work and on sharing this work through appropriate fora. S/he will also represent GOAL as an organisation at the Core group and other health fora in the United States and as required with donor groups. The post holder will be challenged to promote GOALs work and visibility as a health NGO and also to design and develop competitive and innovative health proposals with the field teams
KEY RESPONSIBLITIES:
It is expected that the Development Health Advisor will focus on specific priority health programmes in defined countries however s/he will also be expected to provide technical support and advice to other country programmes as required.
PROGRAMME QUALITY:
• To support on health policy development with a particular focus on supporting the strengthening of health systems and community based health approaches.
• To provide technical support on health issues to defined country programmes team and the Operations team.
• To ensure that gender dynamics, are incorporated into all health activities and is a key component of health staff training and development.
• Participate in health research and analysis with a particular focus on Community Health –ensure that such research and methodologies are disseminated within the organisation.
• Monitor international best practise, innovation and external research and disseminate appropriate research and tools within the organisation.
• Assist GOAL country programmes in developing community health approaches which are based on behaviour change/barrier analysis methodologies and which seek to address the underlying causes of poor health outcomes for communities.
• To ensure that HIV mitigation activities are a part of all health activities and to ensure the integration of nutrition into primary health programming.
PROGRAMME DEVELOPMENT:
• To represent GOAL at selected technical health meetings in the US, with donors such as USAID and to other external agencies as required by Operations Manager/Business Development Manager.
• To assist in donor proposal development in particular for priority countries as required by in country teams and or GOAL Dublin.
• To actively seek out new sources of health funding and support country health programmes to develop health proposals for new and innovative health donors.
• To ensure that HIV is mainstreamed across all GOAL health programmes and that GOAL HIV audit and monitoring tools are being used and understood by country programmes.
HEALTH SYSTEM STRENGHTENING
• Ensure that GOAL programmes are working with MoH structures at local and national level to support health system reforms.
• Apply knowledge and expertise in health policy and programmes to design and develop activities that promote health policy reforms/development.
• Support country health programmes to transition their health approach to ensure that health system strengthening is a key component of their approach.
• Work at the policy level with key decision makers from the host government, participating in strategy development and technical discussions relating to future direction of the health programme.
HEALTH HUMAN RESOURCES:
• Coordinate with other members of the technical team in particular the members of the Public Health sector (which includes WASH, Nutrition and HIV technical staff) in order to develop GOALs integrated approach to programme planning and implementation.

• Document GOAL best practise in health and where possible to publish and or disseminate such documents to external organisation as a way of promoting the visibility of GOALs work in health
• To take the lead on supporting periodic internal evaluations of country health programmes and assist and participate in external health evaluations as required.
• Assist in developing appropriate health monitoring tools and ensuing that those which are in place are being used appropriately and are sufficiently robust to assist in programme review.
EDUCATION AND QUALIFICATIONS:
ESSENTIAL:
• At least Master's/PhD Degree in Public Health or a related subject with some background in health systems strengthening.
• Experience of conducting health research, data analysis and documentation of best practise

Under the direct supervision of the Program Director, the Program Coordinator will coordinate the daily operations of two Ryan White funded performance based contracts. The Food and Nutrition program provides pantry bags, meals, and nutritional counseling/groups to HIV positive adults. The Building Bridges program provides mental health and substance use services using individual, couple, and group counseling treatment modalities. Program Coordinator must be willing to work out of two primary locations throughout the week and additional program locations on a monthly or as needed basis. The Program Coordinator is also responsible for ensuring that both performance-based contracts report services accurately each month and that progress is monitored through data driven supervision.
This position requires knowledge of Microsoft Excel, strong organizational skills, and ability to prioritize multiple tasks. The Program Coordinator must be able to work well in a team-oriented environment within a Harm Reduction service delivery model and have experience working with persons living with HIV/AIDS, who are experiencing mental illness, substance abuse issues, and addiction/compulsive issues.
Essential Job Functions
The following duties are mandatory requirements of the job:
Building Bridges:
• Administrative supervisor for two Mental Health Specialists and MSW Intern(s) to ensure that paperwork is completed within required time frame, encounter logs are completed appropriately, and documentation is filed in the chart to support the encounter logs.
• Work closely with the Clinical Supervisor to assign new referrals to the Mental Health Specialists.
• Monitor caseload size and performance of Mental Health Specialists, ensuring that the program reaches service delivery goals.

• Review chart documentation and encounter logs on a regular basis as a part of the Continuous Quality Improvement process.

• Coordinate pantry distribution schedule.
• Ensure that any changes to the daily schedule are well communicated with clients (such as holidays).
• Maintain and track client service and connection to care and communicate performance to team.
• Oversee the team to ensure that clients attend primary care physician appointments every 90 days, and that staff are completing documentation of this service.
• Monitor client participation and outreach clients who have not been attending the program.
• Complete the case closure process for clients who are no longer attending the program.
• Attend case conferences.
• Provide documentation of case closure for all clients of the Food and Nutrition Program
Other Responsibilities

Education and Certification
Required:

Preferred:
• One year of experience providing mental health and substance use services.

The Yorkville Common Pantry, the largest community based emergency feeding organization in New York City, is seeking an English/Spanish Bilingual Nutrition Education Program Instructor for its Nutrition Initiative for Children and Families (NICF). NICF offershands-on, interactive nutrition education workshops offered to children and adults to improve the overall quality of life of the members and guests of the Yorkville Common Pantry. A strong emphasis is placed on the nutritional value of fresh fruits and vegetables and their impact on healthy living and longevity. The Nutrition Education Instructor will implement the Yorkville Common Pantry's nutrition education curriculum to sites throught Upper Manhattan and the South Bronx. The Nutrition Education Instructor is also responsible for ensuring accurate client data is entered into the client database and that evaluation data is properly entered into the respective spreadsheets and submitted in a timely manner to the Nutrition Education Program Manager. This job is a full-time, exempt position and has a Tuesday through Saturday schedule. Extensive travel is required.
Job Responsibilities
1) Provide offsite nutrition education classes through YCP's NICF program: 10 classes for children using the current curriculum, at least 3 times a year.
2) Maintain all data collection of participants of classes, including monitoring academic progress of individual students and classes.
3) Ensure that evaluation data is properly entered into the respective spreadsheets and submitted in a timely manner to the Nutrition Education Program Manager.
4) Participate in recruitment and registration of participants of YCP's NICF program.
5) Maintain classroom participation with a minimum of 20 participants per children's classes.
6) Take participants to Farmer's Market field trips or a day trip to an Upstate farm from July through November and engage them in a reflection activity.
7) Submit monthly reports on all activity within the instructor's cohort to the program manager.
8) Participate in monthly staff meetings.
9) Work on other program related activities as needed.
Qualifications: Minimum of 2 years' experience working in nutrition education; strong classroom management skills required; A minimum of an AA degree is preferred, however related work experience may substitute for education requirement; Must be computer literate; Knowledge of client tracking programs a plus; Strong clerical skills with typing at least 25 wpm; Must be comfortable working with diverse populations; Must be able to lift 30lbs; English/Spanish Bilingual is required.

Utilizing subject matter expertise regarding kidney disease and treatment options, implements and coordinates all aspects of TOP (Treatment Options Program), a FMS pre-ESRD patient education program, within a designated region. Collaborates with the local management and facility staff to ensure that all pertinent patients have an opportunity to receive TOP education prior to initiating dialysis. TOP patient education includes an unbiased presentation of transplant, home therapies (PD and home hemodialysis), in-center therapy, and palliative care. Markets TOP to physicians, hospitals, and any potential referral source.
Supports FMCNA's UltraCare mission by offering the highest quality individualized patient care and outstanding customer service for chronic kidney disease patients educated through TOPs. Adheres to the FMCNA Compliance Program, including following all regulatory and division/company policy requirements.
DUTIES / ACTIVITIES:
CUSTOMER SERVICE:
• Responsible for driving the FMS culture through values and customer service standards.
• Accountable for outstanding customer service to all external and internal customers.
• Develops and maintains effective relationships through effective and timely communication.
• Takes initiative and action to respond, resolve and follow up regarding customer service issues with all customers in a timely manner.
PRINCIPAL RESPONSIBILITIES AND DUTIES

• Collaborates with the Director Operations or Area Manager to identify and designate area TOP team members.

Ensures that all education provided in group and individual TOP classes are objective and free from bias with respect to all renal replacement therapies.
-Determines team members' roles in TOP education class and trains new TOP team members (RN, SW, RD, MD, PCT) as needed in their role as it relates to TOP;
-May conduct all or part of an education class and/or individual educational session, gathering and submitting data, patient follow-up, and communication with referral source.
-Responsible for choice of educational materials to be utilized for TOP education classes and individual sessions. Obtains Medical Director approval of materials. Orders and distributes TOP materials to area TOP.
-Coordinates an extended service to geographically remote facilities within the designated region by working with the facility staff – Clinical Manager, Social Worker and Dietitian – to provide the opportunity for TOP education for all patients.
-Develops the monthly class schedule and ensures appropriate and timely posting on the UltraCare website.
• Participates in monthly TOP department calls and provides feedback to Corporate Director of TOP in regard to recommendations of new materials and program improvement.
• Responsible for all patient follow up and/or coordination of all patient follow up with TOP team members at the 30-90-180 day thresholds.
-Oversees the appropriate documentation, and communication of follow up activities to referring physicians.
-Communicates with referral sources regarding patient attendance at TOP sessions and follow up communications.
• Responsible for all TOP data collection providing oversight and training of other TOP team members in data collection and submission as needed.
-Ensures appropriate data input into SAP (TOP database) verifying data for accuracy as applicable
-Ensures applicable maintenance and updating of data in database.
-Performs analyses of all TOP related data statistics and reports back regarding findings to TOP team members.
-Identifies trends, variances and anomalies as needed.
-May research issues and make recommendations to TOP team or Director as a result of the analyses.
• Provides reports and analyses to DO/AM regarding outcomes of program. Identifies areas of success and problems or concerns to be addressed pertaining to the program. Analyzes and reports data as needed for Quality Assessment & Improvement (QAI) purposes.
• Coordinates efforts with DO/AM in analysis of local market to explore and prioritize TOP development opportunities to ensure program growth, utilization and effectiveness.
-Markets FMS TOP services to physicians and encourages patient referrals within guidelines established by company policies and government regulations.
-Contacts and communicates with all nephrologists and/or nephrology groups within region on a monthly basis to assess physician issues, concerns, and requests, and to provide feedback on patients seen within the program.
-Provides TOP related educational programs to physicians as needed.
-Maintains knowledge of FMC Treatment Options Program services, promotional materials, and industry educational resources in order to effectively market TOP to physicians and the community.
-Establishes relationship with local renal organizations and affiliates to optimize opportunities for FMS participation in community outreach programs and activities. Examples are: National Kidney Foundation (NKF), American Association Kidney Patients (AAKP)
• Provides TOP information to local FMS facility staff members and affiliated Medical Directors
• Evaluates outcomes of local program. Addresses issues identified, explores and implements improvement opportunities.
• May collaborate with Renal Care Coordinators (RCC) to deliver TOPs education to patients followed by the RCC.
• May collaborate with the Managers of Business Development (MBDs) to provide TOPs education to patients followed by the MBD.
• Other duties as assigned.
