Under the direction of the Director of Clinical Nutrition act as an integral part of the health care team. Assess the nutritional needs of patients and plans for the provision of appropriate nutrition as it pertains to the patients' nutritional needs. Provide nutritional counseling to patients as needed. The registered dietitian is responsible for activities that support patient focused care within their assigned patient units. These include but are not limited to: responding promptly to nutrition consults, providing nutritional care which includes nutrition assessment, evaluation of response to diet therapies, providing diet education and nutrition information to patients' and/or patients families as per department policies.

Principle Duties and Responsibilities:

1.Evaluate and assess patient's nutritional status, review and plan care around prescribed/suggested therapeutic diet.

2.Evaluate and record in patient's chart nutritional status based on dietary intake information, laboratory values, tests, anthropometric measures, clinical signs and other sources when applicable.

3.Suggest appropriate feeding modalities such as enteral or parenteral feedings to enhance the patients' nutritional status.

4.Provide diet therapy recommendations and initiate diet changes when appropriate and according to procedure.

5.Work with patient unit staff to ensure menus are prepared in accordance to procedure and/or recommendations.

6.Instruct groups, patients and/or patient family members in regard to diet therapy to be used during and after hospitalization. Participates in developing various educational materials.

7.Participate as a member of the patient care team, including rounds and discussion of patient treatment plans. Confer with care team members and house officers on orders. Respond to consults, questions and discuss problems.

8.Provide follow-up patient care and intervention as needed or per departmental guidelines.

9.Develop and participate in quality improvement initiatives for the Department of Clinical Nutrition and the care center both for specific patients and families as well as quality improvement initiatives for the overall population.

10.Work with other patient unit staff to recommend and develop plans regarding length of stay and/or expense reduction.

11.Maintain and enhance professional skills by participating in Department of Clinical Nutrition educational programs as well as maintaining external resources.

12.Provide staff development and in-services to care center personnel and newly hired patient unit staff on the principles and application of nutrition, diet and related procedures. Make recommendations on new policies and procedures.

13.Precept dietetic interns and instruct them on specific duties as applied to their assigned rotation. Provide evaluations on students for the next preceptor so that competencies that need improvement may be identified and addressed in a timely manner.

14.Responsible to the Director of Clinical Nutrition in the demonstration of conformity to all legal, regulatory agencies and Department of Clinical Nutrition policies, procedures and guideline requirements.

15.Other duties as assigned.

The Montefiore School Health Program (MSHP) provides comprehensive medical, dental, mental, and community health services to students in 19 public school sites in the Bronx. Our health centers, positioned within the school, provide full access to high quality comprehensive primary and preventive health services regardless of a student's ability to pay.
As a member of the clinical inter-disciplinary team, the Community Health Division (CHD) uses a public health approach to eliminate health disparities facing children living in the Bronx. Through a comprehensive strategy, the Community Health Organizer (CHO) provides leadership and support in developing health-promoting school environments to improve the well-being of all students and their communities. The CHO is responsible for coordinating the effective delivery of classroom-, school- and community-based health promotion and preventive initiatives in cooperation with the school, community partners, government agencies and community members. The MSHP Community Health Division seeks to hire a Community Health Organizer to work in one or two schools to promote the school-based health centers' objectives and community health programs.
Position Summary
On a classroom-, school- and community-level, the Community Health Organizer coordinates the implementation of MSHP developed and/or collaborative health promotion initiatives, conducts health education workshops, facilitates youth development groups, organizes outreach programs and needs assessments, and supports health policy development and implementation to improve student health and well-being. The ideal candidate will be adept at creating and maintaining valuable partnerships within the school, the school community and throughout New York City.
Requirements and Competencies
Associate's Degree in community health, health administration, or related field with a strong focus on public health with three years professional experience conducting health-specific programs and/or policy initiatives involving one or more of the following constituents: children, adolescents, parents, school administrators, teachers, public health and medical professionals.
• Proficiency in basic public health issues, methods and principles. Knowledge in developing and implementing strategies to reduce public health problems.
• Experience conducting outreach to and forming coalitions with Community-Based Organizations.
• Familiar with needs assessments and reporting.
• Excellent organizational skills. Ability to successfully manage competing demands and monitor multiple programs simultaneously.
• Skilled, engaging and confident communicator. Understands the concerns and priorities of diverse audiences.
• Effective team building skills. Able to demonstrate leadership on projects by working independently. Able to work within a team environment and work collaboratively to accomplish common goals and objectives.
• Strong relationship management skills. Experience in fostering and maintaining cross-sector collaborations.
• Communication competence with students in Kindergarten through Grade 12.
• Ability to use data entry and retrieval systems.
• Knowledge, understanding and experience in two of the following public health areas: asthma, reproductive health, fitness and nutrition.
• Able to work evening hours and weekends for community health-related events and programming.
• Able to travel throughout Bronx, NY and work within NYC DOE public schools.
• Proficiency in Microsoft Word, Excel and Power Point.
• Comfortable and experienced in speaking in front of large groups.
• Fluency in Spanish is required for some MSHP sites.
• Salary is in the high 30s with full benefits.

GENERAL SUMMARY
This position resides at Unity's Brain Injury Rehabilitation Unit.

Collaborates with members of the health care team to provide medical nutrition therapy. Identifies patients' level of nutritional risk based on system criteria. Assesses nutritional needs and determines nutrient requirements based on medical history and relevant data. Develops and implements comprehensive nutrition care plans. Continuously evaluates and monitors the effectiveness and outcome of medical nutrition therapy interventions. Educates patients and family members on appropriate food selections and counsels them regarding eating behaviors to accomplish patients goals. Manages the delivery of nutrition care services as needed. Supervises and mentors diet technicians. Actively assesses, evaluates and maintains professional development and competency.
ESSENTIAL FUNCTIONS
Each Employee consistently demonstrates the following essential functions:

Evaluates and interprets data obtained from the patient and/or medical record, including anthropometric and laboratory information, medications, physician orders and from nutrition and medical histories.


Analyzes subjective and objective information to define nutrition problems, goals and interventions to complete a comprehensive nutrition assessment.


Evaluates the appropriateness of nutrition support, and the effectiveness of nutrition care plans, making necessary recommendations in a timely manner.


Documents all aspects of patient nutritional services in patient medical record, following facility/department policy guidelines and governing body standards.


Identifies and documents appropriate criteria to support medical reimbursement. Considers cost, and safety and effectiveness of delivering nutrition services.


Educates both the patient and family as needed, provides nutrition counseling on an individual basis or in a group setting. Facilitates the development of realistic nutritional and lifestyle goals.


Represents Clinical Nutrition Services on multidisciplinary teams, committees, in-service trainings and community events. Provides nutrition consultative services for other departments throughout Unity Health System.


Communicates with health care team, including physicians and other providers regarding patient plan of nutritional care. Make recommendations, requiring nutritional expertise, to all members of the medical team.


Supervises and mentors diet technicians and diet clerks in completing identified tasks and responsibilities. Provides input on their annual performance appraisals.


Conducts and participates in quality assurance programs ensuring that standards and quality initiatives are met.


LTC dietitians responsible for annual assessments, significant change assessments, and high-risk nutritional assessments.


Represents Unity Health System at various community wide events and health fairs.
