NEBHDCo is seeking a self-motivated and committed individual to fill the position for a Community Healthy Food Advocate (CHFA), to play an important role in helping low-income families in New York City live longer and healthier. Through LISC New York City's new Communities for Healthy Food NYCprogram, seeded by the Laurie M. Tisch Illumination Fund, the program will address and expand the urgent need for healthy food access in four underserved neighborhoods citywide.
This three-year initiative will:
• Incorporate healthy food access, programming, education and outreach into affordable housing developments and existing community services for residents.
• Educate residents about nutrition, food preparation, gardening, community based healthy food resources and overall health.
• Create new and/or improved healthy food venues in vacant or underutilized community or commercial space.
The CHFA will work very closely with NEBHDCo's Community Programs staff, community residents, partner nonprofit organizations, public agencies and businesses, primarily in the Bedford Stuyvesant community. The CHFA will have a close understanding of the culture and needs of the neighborhood, and be able to implement a range of strategies to increase access to healthy food and promote healthy eating among residents. The CFHA will work at a community-wide planning as well as localized programmatic level, as part of NEBHDCo's expanding pantry and urban farming operations.
Established in 1985, Northeast Brooklyn Housing Development Corporation (NEBHDCo) is committed to the preservation, development and management of affordable housing and homeownership opportunities; community and economic development initiatives and human services that effect social change in Central and East Brooklyn. NEBHDCo owns and operates 92 buildings comprising over 1,000 units of affordable housing. Our portfolio also includes 17 commercial storefronts and 8 gardens.
NEBHDCo, through its Golden Harvest food pantry, has been a New York City Emergency Feeding partner agency since August of 1993. We currently feed 700 families twice a month at our current location at 745 Lafayette Avenue in Bedford Stuyvesant. In the summer of 2013 we are moving to a new location at 376 Throop Avenue. The new pantry will operate on the supermarket style client choice model. With this expanded facility we plan to double our client capacity, provide more fresh food and offer programming for healthy food and living.
Work Hours
M-F, 35 hour work week between 9am and 5pm; occasional evening and weekend hours
Essential Duties and Responsibilities
• Working with partner organizations (including City Harvest, NYC DOHMH/Brooklyn District Public Health Office and Bed-Stuy Campaign Against Hunger), review existing healthy food and related public health needs assessments and asset mapping; determine gaps in services, programs, and physical infrastructure;
• Define the catchment area for NEBHDCo's Healthy Food Access project from this review of existing assessments, and in consideration of NEBHDCo's physical and programmatic assets: housing properties, commercial spaces, community spaces, community gardens, food pantry, and related tenant and community program activities;
• Work with City Harvest to update or expand upon their Bed Stuy healthy food asset map to identify additional resources and gaps in healthy food access physical infrastructure and related programs and services, within the NEBHDCo catchment area;
• Develop a summary needs assessment and map (with LISC support) for the NEBHDCo project, with a related plan of action for strategic connections, interventions and supports related to Communities for Healthy Food NYC program components. This may include opportunities for developing more healthy food retail outlets, but will certainly include the program elements below.
• Manage the development and scheduling of NEBHDCo's new nutrition education and healthy living programming using outside partner agencies. Further develop NEBHDCo's healthy food education and fitness partnerships. Support, promote and leverage existing outside community education efforts to improve healthy eating habits and behaviors among local residents, including youth. Develop or expand upon a healthy food outreach campaign. Work with NEBHDCo staff and partner agencies on community outreach and outcomes tracking for these efforts.
• Assist with oversight of food gardening from four NEBHDCo urban gardening/farming sites, especially around harvesting and the distribution of produce at the food pantry. Working with NEBHDCo's urban farming educator and using the Farming Concrete model, track the volume of all NEBHDCo-grown food production and distribution.
• With the expanded capacity of the new pantry, track the increase of fresh food availability and distribution from City Harvest, Food Bank and other current sources; track pantry client interest in the new options.
• Seek additional alliances with key stakeholders, strategic local and city-wide partners, government agencies and other support organizations, particularly within the community health or food justice sector, to create linkages to make affordable healthy food and nutrition education a priority;
• Participate in Communities for Healthy Food NYC partner group meetings on progress, sharing of best practices, and brainstorming solutions to challenges;
• As is applicable from the NEBHDCo assessment summary, assist with a commercial and community facility assessment to decide strategic locations to develop or expand healthy food projects;
• Assist with identifying other financial and technical resources in order to support programmatic investments and identify lending opportunities as applicable;
• Establish impact measures for the project, working with LISC and potential additional evaluation agencies to collect data for program evaluation.
Qualifications:
• MS/MA or equivalent with a major in food studies, public health, public administration, social/human services, sociology, urban planning, public policy or a related field
• At least 2 years' experience with urban food systems initiatives

IMPORTANT:

Candidate must be willing to work at least 30 hours or more per week, including 2 evening shifts per week. Each shift is between 6 and 10 hours. Our normal office hours are Monday through Thursday, from 9am until 9pm and Friday, from 8am until 4pm.

The program, known as La Nueva Esperanza, provides effective comprehensive food and nutrition services to HIV positive Latino and African American residents living inBrooklyn. La Nueva Esperanza program, solely funded via this Ryan White Title I contract, identifies clients who are or are at-risk of malnutrition, hunger, and are in need of food and nutrition counseling services. Eligibility requirements are addressed through an intake/assessment process. Each client's medical history, knowledge of nutrition and HIV/AIDS, and their ability to prepare and/or access nutritious meals are assessed. Barriers that may lead and/or have led to hunger, malnutrition, poor eating habits, side effects to medication, and medical illnesses are identified and client-specific goals are defined. To achieve these goals, La Nueva Esperanza contract-funded staff provide nutrition counseling and education, congregate and home delivered meals, and pantry bag distribution to assist clients understand the importance of nutrition, eliminate or reduce barriers that contribute to hunger, malnutrition, non-adherence to medication, and identify strategies that promote healthy eating habits. Services are provided in a culturally competent manner, acknowledging our clients' diverse beliefs, values, and backgrounds. Services are provided in English and Spanish.

Client enrollment occurs via the program's recruitment efforts. The majority of clients are former clients of the agency. Upon enrollment, a contract-funded staff member conducts an intake/assessment. Each client signs a Service Plan outlining individualized short and long-term goals identified via the assessment process. Food services – congregate meals, home delivered meals, pantry bag distribution (FRIDAY's ONLY)– are available five days a weeks with most clients accessing about six service encounters per month. Nutrition counseling and/or nutrition group education are provided monthly to assist the clients achieve their Service Plan goals. Reassessment and Service Plan development occur on a semi-annual basis. When the client has achieved the goals set in their Service Plan, they receive any necessary referrals and case closure occurs; this will occur by 18-months, an additional 2 months may be required for clients who regress; at the time another assessment will be conducted to establish referral.

Comprehensive Nutritional Assessment
ALL clients who have nutritional needs must receive a nutritional assessment. The Registered Dietician is responsible for thoroughly assessing client's nutrition-related needs. The Assessment must be documented.

Screening & Comprehensive Treatment Plan Development
The program will ensure that a coordinated plan is in place to address the client's food and/or nutrition-related issues. The Registered Dietican must screen clients and document in a Comprehensive Treatment Plan the food and/or nutrition needs of the client and goals for promoting independence from the program (by month 18). Additionally, the plan must address the client's primary care status and ensure that clients not in care are linked to a primary care provider. The Plan must have signatures of the client and Registered Dietician and every item on the treatment plan needs a prospective date of completion attached to it and anticipated resources required

Reassessment and Comprehensive Treatment Plan Update
Registered Dietician will meet with the client to provide a reassessment every six months during enrollment. Client's progress toward achieving goals set in his/her Comprehensive Treatment Plan will be reviewed. The plan will be updated to reflect any changes identified during the reassessment.

Verification of Connection to HIV Primary Care
The program staff must ensure that the client is enrolled in HIV primary care and must verify with the Primary Care Provider of the engagement in care.

Linkage to Care
If the client is not in care, the program staff must link the client to a provider, make the first appointment for the client to meet with the provider, and verify that the client has a primary care visit within three months of enrollment in the FNS program.

Nutritional Counseling w/o supplements
Nutritional counseling involves provision of information, guidance on management of co-morbid conditions and treatment adherence as it relates to nutrition. The counseling will also involve distribution of educational materials and follow-up to help the client make and maintain the needed dietary changes. Counseling sessions should be tailored to the needs of the individual client. Diet and nutrition planning must follow standard nutritional practices. This service is provided by the Registered Dietician.

Nutritional Education Group
Group nutritional education sessions include the provision of information (oral, written, or audiovisual) that is designed to improve client knowledge and understanding of nutritional issues and of the relationship between nutrition, illness and treatment. Groups are conducted by the Registered Dietician.

The RD will be considered a Consultant. No PAID Benefits. They are required to work 8 hours a day/ 16 hours a week/ 64 hours a month. We service approximately 100-150 individuals per year. Monday and Friday ---- $30/hour.
