Background: The Brooklyn DPHO is a community-based arm of the Department of Health and Mental Hygiene that targets resources and programs to neighborhoods in North and Central Brooklyn of high public health need, including Bedford Stuyvesant, Brownsville, Bushwick, and East New York. In September 2010, the DPHO launched Healthy Schools - Brooklyn to assist public schools in in developing and implementing school wellness policies to promote nutrition, physical activity, and tobacco control.

Internship Project: Work with 3 schools on comprehensive wellness policies by identifying a school wellness or partnership committee and assist schools in developing monthly committee meetings to launch school wellness policy work. The intern will assist team and school stakeholders in completing assessment tools to identify current practices and policies around nutrition and physical activity. Following completion of assessment, the intern will work with schools to implement NYC DOE Wellness Policies and assist schools in pursuing resources that will increase physical activity time and opportunities for health eating.

Location: Intern will be based at the Brooklyn DPHO in Bedford Stuyvesant. Intern will work with 3 schools in Bedford Stuyvesant, Brownsville and/or East New York.

Tasks and Timeline: From September 2013 to January 2014, intern will work with Department of Health staff to:
 Contact participating schools and identify existing school wellness leadership team and/or meet with school administration to develop school wellness team;
 Organize monthly meetings of school wellness team to discuss wellness policies and complete assessments;
 Support the school in adopting and implementing the NYC DOE Wellness Policy based on interests and strengths identified in assessments;
 Attend internship meetings with the Healthy Schools Brooklyn Coordinator and report activities with schools and other stakeholders
 Provide brief summary report to DPHO and enter schools data into tracking database.
 By the end of January 2014, summarize findings of completed assessments, present back to wellness team their accomplishments over the semester in implementing wellness policies that increase healthy eating and physical activity in the school and offer feedback to the school regarding their next steps

Qualifications & Experience: The ideal applicant would be a graduate-level student, with preliminary training in community public health and community outreach. Certain activities would require applicant to speak persuasively on health topics. Familiarity with public transportation is necessary. Fluency in Spanish is not required, but a plus.

Job description

RESPONSIBILITIES: The Health Coordinator will report to the Director on-site.
-Maintain complete, current, and accurate case records including case notes, assessments, demographic data, and medical/health information including immunizations and health screenings.
-Arrange for medical, dental, vision and hearing screenings, examinations and follow-up care as specified by the New York City Department of Health and Mental Hygiene and the Head Start Performance Standards.
-Secure doctor's notes and other of medical documentation for children who have been absent with communicable diseases and notify the Health Department as required.
-Inform parents and staff of health resources in the community and establish links with community health groups.
-Assist families in securing medical and health related services.
-Establish Health Services Advisory and Nutrition Committees (recruit members including community partners, health professionals, program staff, consultants, and parents) and coordinate meetings several times a year in order to address health and wellness needs of the families served by the program.
-Facilitate health related workshops for parents and program staff throughout the program year in conjunction with health and nutrition consultants.
-Provide/make available information and resources to staff in order to promote health-related aspects of program and improve staff knowledge and skills especially in the following areas: communicable diseases; emergency procedures; maintenance of staff health; and universal precautions in child care.
-Educate teaching staff and children on dental, health, and nutrition topics including routine issues such as tooth brushing, hand washing, and overall good-health habits and conduct observations of staff carrying out health and safety procedures.
-Manage and monitor computer-based tracking system to provide accurate reports on children's health status.
-Collect accident/incident reports and log information into tracking system, including information for participants who were excluded from the program due to illness.
-Work with inter-agency departments to coordinate annual health fair.
-Perform routine checks of First Aid kits, medical/dental supplies, and replenish/restock supplies as needed; ensure that all required health postings, signs/documentation, and First Aid Kits are visible in classrooms.
-Provide families with timely and appropriate referrals and subsequent follow-up for health, social services and disabilities services (as it may be related to any health concerns).
-Attend and participate in social service team meetings, agency staff meetings, Orientations, Open Houses, parent training/workshop sessions (may facilitate or co-facilitate), staff development sessions, and other scheduled events.
-Participate in community-building activities sponsored by program and agency, and encourage participants to support these activities.
-Proficiency in office computer software and data management systems.
-Other duties as assigned
QUALIFICATIONS:
The Health Coordinator must have at minimum; a bachelor's degree and/or a minimum of two years of experience working with families or in a social service/medical setting. Flexibility and commitment to quality education programs are required. The Health Coordinator should possess cultural sensitivity and awareness, and communicate effectively with families and collateral sources. The Health Coordinator must have excellent time management, organizational and communication skills. The Health Coordinator should maintain the integrity of the organization at all times.

JOB REQUIREMENTS

• Registered Dietitian in good standing with the Commission on Dietetic Registration
