Following the Housing First model, Lantern Community Services (LCS), the social service affiliate of Lantern Organization, provides a complement of counseling and supportive services to help residents stabilize their lives, maintain their housing stability and advance towards their individualized goals, including financial independence.
Organizational Role:

This newly created position provides an opportunity for a performance-driven professional to fully integrate our health and wellness outcome priorities into all levels of LCS programming. The Director will have demonstrated experience implementing evidence-based practices and administering programs in compliance with challenging regulations and policies and a holistic and recovery-oriented approach to fostering health and wellness.

Successful execution of this program necessitates the ability to cultivate and sustain relationships with quality program partners; to have comprehensive knowledge of complex public benefit regulations and to provide strong staff leadership and oversight.
The Director of Health and Wellness reports to the Executive Director. This position is exempt.
Program Management and Partnership Development
• Based on evidence-based practices and evaluation of our existing services and outcomes, develop a comprehensive health and wellness program to be implemented across all Lantern buildings.
Adapt engagement strategies and program offerings to different client populations, including families, youth aging out of foster care and single adults with a range of challenges.
Lead team of health and wellness coordinators and leverage their respective expertise and skill sets to offer group workshops and individualized support on topics ranging from stress and anger management to grief and trauma, wellness self-management and sexual health, as well as community building activities.
• Establish and sustain partnerships with providers and community partners to enrich health and wellness opportunities for our clients.
Develop LCS's program structure for launching and operating new initiatives and assessing how our programs can benefit from new developments in the health care system such as New York State's Health Homes care management model.
Outcomes Performance and staff supervision
• Hire, train, supervise and develop a program team with a strong sense of accountability for achieving outcomes for residents and complying with Lantern and funder regulations and requirements.
• Ensure all case records (electronic and paper) are accurate, comprehensive and up-to-date.
• Stay current on best practices and field trends.
• Meet regularly with Clinical and Program Directors to facilitate integration of health and wellness objectives and programming into all supportive services.
