Job Title: Clinical Care Manager

·       CDE required. Current RD license required

·       Bi-lingual preferred (English / Spanish). Harlem Residence preferred.

·       Department: Health Coaching team Reports to: Director of Health Coaching

·       Position Type: Full-time FLSA 

·       Status: Exempt (salaried)

·       Location: Harlem, NY 

·       Experience: 5-10 years clinical experience;2-3 years Case Management or Care

·       Coordination experience
