Company: Urban Health Plan Inc Title: Nutritionist Registered Dietitian (RD) desired to provide 1:1 group nutrition education and counseling to high-risk patients and families in an outpatient setting. The nutritionist will be responsible for providing outreach activities and therapy to patients of unhealthy and/or the chronically ill including patients of various age ranges regarding potential of diseases associated with unhealthy nutritional choices. Along with working closely with multi-disciplinary team to develop innovative ways to educate patients, including but limited to facilitating food demonstrations. Must be flexible and willing to travel to different site locations in the South Bronx. Bilingual in Spanish Required, flexible hours, highly motivated with great communication skills preferred.
