EPODE’s goal is to enable community stakeholders to implement effective and sustainable strategies to prevent childhood obesity and NCDs at the local level. In communities implementing the EPODE model, we have seen promising results. Between 2004 and 2009 in a community in Northern France, a 10% reduction in the prevalence of overweight and obesity demonstrated the efficiency of the EPODE methodology. More recently, in Belgium, the Viasano community-based programme (CBP) has seen a reduction of 22% in the prevalence of overweight in its pilot towns and between these pilot towns and comparison towns of the French community. Minimum Qual Requirements

2. Education and/or experience equivalent to "1" above. Education may be substituted for experience on the basis that each 30 graduate semester credits from an accredited college in hospital administration, public health, public administration, business administration, management or administration can be substituted for one year of non-managerial experience up to a maximum of 60 semester credits for two years. However, all candidates must have a minimum of a baccalaureate degree and 18 months of managerial experience as described in "1" above.

Edible Schoolyard NYC Spring Garden Volunteer Program About Edible Schoolyard NYC Edible Schoolyard NYC transforms the hearts, minds and eating habits of young New Yorkers through an integrated seed-to-table education. Position Description The Garden Volunteer Program is a weekly commitment that runs mid-February to mid-May during the fall semester at P.S. 216 in Gravesend, Brooklyn. We ask volunteers to make sure they can attend at least 9 of those 14 weeks. Working under the supervision of the Garden Manager and Assistant Garden Manager, volunteers assist with garden maintenance; cultivate food grown for the kitchen classroom and farm stand; and model positive behavior in the teaching garden, a place of wonder and respect for the environment and the community. Semester-long volunteers have the opportunity to develop and share skills related to growing organic food in an urban environment, and will work alongside equally enthusiastic advocates of edible education. This is not a teaching position. Job Summary: The role of the Health and Wellness Program Associate is to conduct training for home-based child care providers in the areas of nutrition, health and wellness, and physical education. The Program Associate will work in close collaboration with the Nutrition, Health and Wellness Coordinatorto ensure the organization meets its targets in accordance with its contractual obligations under the Eat Well Play Hard program, CACFP and other internal performance measures. Additionally, the Program Associate will support the work of our Research, Policy and Evaluation Department by gathering data through surveys, observations and case notes, and inputting results into tracking systems.
Responsibilities:
• Conduct training workshops for home-based child care providers consistent with the Eat Well Play Hard and CACFP programs, which promote nutrition and physical activity in early childhood, with assistance from the Nutrition, Health and Wellness Coordinator.
• Conduct home visits to child care providers enrolled in the Eat Well Play Hard and CACFP programs.
• Assess home-based child care programs for compliance with Eat Well Play Hard and CACFP program standards, identify areas for improvement, and work with providers to develop and achieve related goals.
• Complete administrative work including case notes, filings, mailings, and other required program documentation.
• Data entry of nutrition, health and wellness information into various tracking systems, as needed, for compliance with contractual obligations and the organization's internal program goals.
• Assist Nutrition, Health and Wellness Coordinator with developing and implementing new health and wellness initiatives.
Qualifications:
• A minimum of a bachelor's degree, preferably in a health or education-related field; strong interest in these fields is required.
• Bi-lingual in English and Spanish with excellent communication and presentation skills.
• Ability to work well with both adults and children.
• Ability and willingness to travel to and visit day care homes in New York City (primarily in the Bronx), which may involve climbing stairs.
• Have demonstrated organizational skills.
• Familiarity with Microsoft Office applications.
• Available to work evenings and some Saturdays.

Job Description
• Perform nutrition assessments and reassessments daily as per assigned unit and in order of risk level, admission date and time as per the current policy of the department.
• Visit patients to obtain a detailed nutrition history summary and determine their nutritional needs.
• Perform meal rounds to observe patient’s acceptance of meals before completing a reassessment.
• Provide nutrition education and counseling to patients (and family if appropriate) identified at nutritional risk prior to discharge.
• Participate in discharge planning rounds and interdisciplinary team rounds in assigned unit to discuss the plan of care and recommendation with the medical team.
• Print out daily work-lists, MD referrals and nursing referrals daily on assigned units to determine which patients need to be seen.
• Participates in the formulation and review or departmental and hospital wide policies and procedures for the operations of the department.
