Dietitians on Demand LLC is seeking a Registered Dietitian for a three-month consulting assignment at a PACE program facility in the Trenton, NJ area. This position needs 40 hours per week of coverage during regular daytime hours.  This contract pays a competitive hourly rate and requires professional liability insurance.  Anticipated start date is February 23, 2015.

 

Responsibilities:

Coordinate nutritional care of residents and patients by completing nutritional assessments at home or in facility
Monitor participant progress and outcomes through assessments of care provided, including any needs for treatment plan revisions.  Participate in care planning meetings with other members of the interdisciplinary team
Provide nutritional counseling and education for residents and family
Document assessments and changes in participant conditions by completing all required PACE documentation and ensuring compliance to Agency standards and policies

 Requirements:

Candidates must be Registered by the Commission on Dietetic Registration
At least one year of experience as a professional dietitian working with a geriatric population in a long term care or home care setting
Demonstrated adaptability, flexibility, problem-solving skills
Strong communication and interpersonal skills
Experience with electronic charting, preferably Mediture/TruChart, and the Nutrition Care Process
