LPHI seeks high caliber professionals to lead its growing portfolio of health initiatives that further the triple aim of improving quality of care, improving population health and reducing health care costs with a focus on the safety net population. LPHI is a dynamic, independent nonprofit organization that designs, leads and evaluates innovative clinical transformation projects across Louisiana and the Gulf South to help accelerate the shift from “fee-for-volume” to “fee-for-value” health care services.

We are currently recruiting for the following three positions to lead LPHI’s pioneering work with primary care clinics, federally qualified health centers (FQHCs), hospitals, primary care associations, and other health system partners in Louisiana and across the Gulf Coast.

Senior Health Systems Strategist is responsible for the overall strategic and tactical planning, partnership development and management (or business growth) of LPHI’s internal and external health systems projects and activities. As a senior-level, executive and member of LPHI’s senior leadership team, The Strategist stays abreast of national and state trends and incorporates innovative best practices into LPHI’s business model to advance LPHI’s mission of “health for all”. This position also collaborates closely with other LPHI senior executives in assuring that health systems, community health, informatics, analytics, and research activities at LPHI are coordinated and integrated seamlessly. Requirements:
•   Masters degree in Public Health, Health Systems, Business Administration, Economics, Health Administration, or equivalent required
•   Doctor of Medicine or Doctor of Philosophy degree in a relevant field preferred.
•   10+ years indicating progressively challenging responsibilities/positions with at least 5+ years in a leadership position
•   Experience of leading large, complex programs with multiple stakeholders and covering a broad area of health systems
•   Hands-on clinical experience preferred
•   Demonstrated ability to develop and manage partnerships and projects

The Clinical Transformation Portfolio Director will advance LPHI’s innovative work with primary care clinics, federally qualified health centers (FQHCs), hospitals, primary care associations, and other components of the health system in Louisiana and across the gulf coast. S/he will serve as the Project Director of the complex and dynamic Primary Care Capacity Project (PCCP) under the Gulf Region Health Outreach Program (GRHOP) and assure quality, timely delivery of other project objectives under the portfolio. All LPHI Portfolio Directors act as subject matter experts and assure alignment across LPHI’s diverse projects. Requirements:

•   Master’s degree in related field
•   7+ years of experience in progressively complex roles in health care or health systems work
•   Prefer 3+ years working at a leadership or supervisory level in a health care organization, such as an FQHC or a safety net hospital, with a focus on operations, clinical quality and/or business development
•   Demonstrated ability to manage large, complex programs
•   Successful experience providing coaching and/or technical assistance to clinical teams
•   Thorough understanding of the use of information technology for quality improvement, appreciation of the value of data analytics, and commitment to patient-centered approaches 
•   Extensive direct experience working with executives in hospital systems, public health departments, and payer organizations is preferred
•   Comprehensive understanding of the reimbursement environment for primary care and challenges faced by the safety net healthcare organizations is preferred
•   Track record of successful grant writing/proposal development in cross disciplinary teams

The Practice Transformation Network Director will lead and oversee the implementation of LPHI’s proposed four year proposed Transforming Clinical Practice Initiative (TCPI) project if funded by the Centers for Medicare & Medicaid Services (CMS). In collaboration with a dynamic set of partners across Louisiana, Mississippi, and Alabama, the Director will lead the multi-state Patient Transformation Network consisting of a combination of practice types including Federally Qualified Health Centers, university-affiliated multi-specialty practices, hospitals, and private practices among others. Using a robust combination of direct clinical support, learning diffusion methods, health information technology, and business analytics/informatics, the Network will help practices progress through CMS’ five phases of transformation, achieve measureable patient outcome improvements, and save costs. Requirements:
•   Masters degree in Public Health, Health Systems, Business Administration, Economics, Health Administration, or equivalent required.
•   A clinical degree (medical, nursing, pharmacy, etc.) is desirable. 
•   10+ years indicating progressively challenging responsibilities

•   Preference given to candidate with 5+ years working at a leadership or supervisory level in a health care organization or system 
•   Demonstrable expertise in large health systems and clinical practice transformation
•   Proven expertise in quality improvement and population health management
•   Experience of leading large, complex programs with multiple stakeholders and covering a broad area of health systems
