JOB DESCRIPTION

JOB TITLE: CLINICAL DIETITIAN

JOB SUMMARY:  Under the direction of the Assistant Director/Chief
Dietitian plans and delivers nutrition care of residents/patients in accordance with the physicians' diagnoses and requirements of regulatory agencies. Functions as a member of the multi-disciplinary health care team to screen, assess and care plan for all residents and patients.

DUTIES AND RESPONSIBILITIES:

1.   Evaluates medical records to obtain necessary medical
information to determine resident/patient nutritional status for screening, completing nutritional assessment and completing a care plan.

2.   Documents recommendations in the resident/patient medical record according to department and facility policy to communicate with physicians and the health care team.

3.   Participates in Comprehensive Care Plan meetings to provide nutrition information regarding patient/resident nutritional needs.

4.   Evaluates effects of nutrition intervention individual resident/ patient nutritional status to determine need for further intervention or revision of nutrition care plan.

5.   Performs a variety of related duties as assigned which include but are not limited to meal rounds, caloric intake, diet education, modification of resident menus, and computer updates.

CLINICAL DIETITIAN

6.   Ensures that residents/patients receives the appropriate diet as per physicians order and resident's/patients preferences, tolerance, and or allergies.
7.   Attends interdisciplinary meetings as scheduled including ethics rounds, discharge planning etc.

8.   Conducts inservices/presentations as requested to health care professionals, employees and community to enhance understanding of the role of nutrition in health and to promote role of the Dietitian.

9.   Establishes and maintains effective working relationship with medical, nursing, social work, speech pathology, occupational and physical therapy and pharmacy staff to provide a united approach to resident/patient care.

10. Establishes and maintains effective working relationships with Food and Nutrition Services.

11. Participates in ongoing departmental performance improvement program.

12. Maintains accurate records as directed by Assistant Director (i.e. Clinical productivity records) to meet regulatory agency guidelines and assist in statistical analysis of data.

13. Assist with on-the job training of Dietetic Technicians, Dietetic AP4 students and other employees to ensure an understanding of departmental policies and procedures and guidelines for clinical care.

14. Participates and coordinates research activities in special interest areas relating to nutrition.

15.  Performs all other related duties as assigned.

 SPECIALIZED SKILLS AND COMPETENCIES

1. English writing skills developed to a high degree to enable communications in the medical records and in other health related documents.

2. Knowledge of clinical nutrition as appropriate for geriatrics plus the acute and subacute population.

3. Knowledge of the dietary computer systems or prior experience with computers.
4. Ability to effectively communicate with residents/patients and other health care professionals.

5. Ability to be flexible and adapt easily to changes in schedules and work environment.

6. Ability to respond appropriately to resident(s) with Dementia behavioral symptoms.

AGE RELATED SKILLS AND COMPETENCIES:


1.           Ability to assess the nutritional status and plan appropriate care plans to meet the needs of the elderly and adult population.

JOB QUALIFICATION:

Education:  Bachelor’s Degree required, Master’s Degree in Food &
Nutrition and Registration with the Academy of Nutrition and Dietetics preferred.  RD or RDE status with the completion of the RD exam within 6 months of hire.

Training: One year experience in working in a long term care
setting preferred.

Maintains continuing education credits as required by department policy and/or to maintain Registration status through the Academy of Nutrition and Dietetics.
