St. Barnabas Rehabilitation and Continuing Care Center (SBRCCC) is a long-term care facility situated in the Belmont section of the Bronx.  SBRCCC is in need of a Clinical Dietitian to join their interdisciplinary care team.  Candidates can be RD or RD-eligible.

Job Description:
Include but not limited to: 
- Perform comprehensive nutritional assessments for patients with all levels of clinical   

  conditions.
- Implement, evaluate, monitor and update individualized care plans.
- Complete MDS 3.0 documentation per regulatory guidelines.
- Perform meal rounds and meal audits.
- Participate in care plan meetings.

- Participate in wound rounds.
- Maintain open lines of communication with food service personnel, health-care team,

  patients, and their family regarding plan of care.
- Provide up-to-date education on therapeutic diet and food/drug interactions to

  patients, family and staff.
- Keep up-to-date with current standards of practice in the nutritional field. 

- Precepts dietetic interns as needed.

 

QUALIFICATIONS:

 A.   Education Required: B.S. in Food and Nutrition/Dietetics

B.   License(s) Required: Membership in the Academy of Nutrition and Dietetics.  Registered Dietitian or eligible to take Registration Exam.  Must take RD exam within 3 months of entering position and must past within 1 year.

 C.  Experience/Skills Required: Ability to speak Spanish desired, but not   required. Ability to communicate with Resident/family members and interact with other members of the Healthcare team.  Experience with AIDS/Geriatric/Ventilator-Dependent & Dysphagic population is highly desirable.

The Nutritionist/Dietitian has overall responsibility for the development and monitoring of all nutritional services of the ADHC. These services include menu preparation, consideration with food preparation staff, and the selection of client meals and snacks. Additional responsibilities include providing nutritional/therapeutic counseling and ongoing nutritional education to partici-pants of the ADHC, AmidaCare and primary care patients within specified time frames, supervising and monitoring the nutritional status of clients through written assessments, observing food intake.

Responsibilities
1. Completes an individual client nutritional assessment within five (5) visits and in a period not to exceed thirty (30) days after admission on all clients of ADHC and every ninety (90) days thereafter.
2. Completes an individual client nutritional assessment on every primary care patient not to exceed thirty (30) days after initial visit to primary care.
3. Evaluates nutritional status based on developed protocols and standards, develops individu-alized nutritional care plans, and maintains a nutritional record in clients' chart.
4. Supervises nutritional planning, food production/preparation, dietary modifications and make all necessary adjustments on weekly menu of the ADHC with food service staff
5. Contributes to initial and ongoing comprehensive care planning and attends comprehensive care planning meetings, morning rounds when on site.
6. Develops and facilitates group sessions in consultation with interdisciplinary team to address nutrition, menu planning, cooking, and current trends in diet therapy to clients in program.
7. Provides in service education to the program staff and volunteers.
8. Acts as liaison to external care providers in regard to possible interaction of medical condi-tion, drug treatment, and nutritional status.
9. Maintains client caseload for purpose of coordinating care both within program and with outside case management providers.
10.Supports and participates in advocacy efforts to end the twin crises of homelessness and AIDS.

Education/Skills Requirements
The candidate must be registered as a Dietitian from a school accredited by the American Dietetic Association with a Masters Degree from an accredited college or university. Two years clinical experience in a health setting preferred. Experience in working with HIV/AIDS population desirable.

NY Common Pantry, the largest community based emergency feeding organization in New York City, is seeking a charismatic and outgoing Intake Manager/Nutritionist to manage the Intake and Nutrition Education operations of its new Commodities Supplemental Food Program (CSFP) for Senior Citizens at its permanent location in the Bronx and mobile sites across the Bronx, Manhattan, Brooklyn and Queens. Comply with State directives Senior Farmers Market Nutrition Program to provide necessary check booklet security measures including procedures for receipt, inventory, and storage in accordance with the CSFP Manual.

Ensure that quality service provision is provided and address all customer service issues that arise during intake and nutrition education at permanent and mobile sites.

Establish a protocol for wait listing eligible participants when instructed by the Director of Food Programs to do so.

Establish and implement a system to ensure confidentiality of participant records.

Submit all required reports to the Director of Food Programs in a timely manner,

Assist with other program-related activities as needed and assigned by the Director of Food Programs.

The facility will provide 30 units of supportive housing to homeless or at-risk of homelessness, single adults (age 18 and over) diagnosed with HIV/AIDS, serious mental illness (SMI) or who are diagnosed as mentally ill and chemically addicted (MICA), currently known as persons with co-occurring disorders. CAMBA, Inc. is the on-site services provider at Gabriel House and provides case management and 24/7 security.

The person filling this position is expected, under close supervision, to: (1) identify patients with nutritional risk using scoring guidelines and initiates referrals for dietetic assessment; and (2) institute basic nutrition plans including individualized meal plans.

MINIMUM EDUCATION/EXPERIENCE REQUIRED: Registered dietician with two years experience.

 OTHER REQUIREMENTS: Knowledge about, understanding of, and ability to work closely with, persons with HIV/AIDS and related issues.

 Position Summary

Registered Dietician

Evaluates and provides ongoing assessment of patient's Nutritional Therapy needs, reports findings / patients' status to the Community Health Nurse. Carries out the prescription of the physician for the individual patient in the PACE Centeror at home; prescriptions may include (1) diet evaluation, (2) diet diary, (3) diet review.

 Participates in interdisciplinary team meetings to discuss patient's progress in developing patient's plan of care. Establishes frequency of visits based on physician's order and in collaboration with the Community Health Nurse, documents patient's progress in the medical record at each patient contact and completes appropriate program summaries. In-services staff and families/caregivers in treatment program.  Assists in Utilization Review and Performance Improvement activities as requested. 

Candidate must have Bachelors Degree from an accredited Dietetics program along with New York State Licensed as a Registered Dietician and at least one year experience as a Nutritional Therapist, preferably in a Community or Outpatient settings. Home Care experience preferred.
