Diabetes Educator/Nutritionist
SUMMARY:  Diabetes educators are highly skilled professionals integral to the multi-disciplinary medical team approach. They provide diabetes care and self-management education to Bridges patients, access staff and access’ general patient population to meet the goals and objectives of the Bridges to diabetic care pilot program. Community dieticians work in the public arena, promoting public health and educating the community on healthy eating practices. These individuals are often responsible for public health advertisements regarding dietary habits, and provide educational workshops or one-on-one counseling to promote healthy eating. This position combines both types of expertise and skills. The Diabetes Educator/Nutritionist will serve as primary point of contact for the program and will work with the Medical Director, to coordinate the Bridges program.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Diabetes and Nutrition Education
• Works with CMO and Behavioral Health Specialist(s), to develop on-line diabetes self-management curricula tailored to the developmental and health literacy needs of Group A and Group B project participants, as well as the needs of the general Access medical population. The group sessions will focus on patient and caretaker health literacy in regards to diabetes and will help participants understand the relationship between nutrition, activity level, emotional and physical status, and medication compliance, and then respond appropriately and continually to those factors to achieve and maintain optimal glucose control.
• Responsible for creating and updating diabetes specific educational information on internet based programs including: Access social media sites (i.e. Facebook, Twitter, and YouTube) and website (i.e. Video links).  Utilizes internet based application such as Skype to speak with pilot participants.
• Collaborates with clinicians to identify and connect with patients in need of nutrition counseling.
• Establishes treatment plans and communicates with patients regarding program compliance.
• Meets directly with patients to identify educational needs; provide individual instruction, utilize informational handouts/booklets, or refer to community resources as needed.
• Identifies ways to improve care of patients, and make recommendations to the CMO and P&T Committee; Participates in various patient program committees; Performs research and recommends ways to expand and improve nutrition programs; Performs other duties and responsibilities as assigned.
2. Program Coordination
• Responsible for creating and disseminating program-specific text messages to pilot participants:  (including appointment reminders, group session reminders, weekly information that repeats and reinforces information shared at last group session, video links, reminders to eat healthy, etc.).
• Develops and recommends new or revised program goals and objectives.
• Works with CFO and Director of Operations to monitor and approve program expenditures ensuring that budget allocations are not overspent.
• Coordinates activities of program with inter-related activities of other programs, departments or staff to ensure optimum efficiency and compliance with appropriate policies and procedure.
• Prepares periodic reports records on program activities, progress, status or other special reports.
• Works with the CMO to promote the program to Access patients in order to recruits project participants. Reviews applications or other program documents in conjunction with the CMO and Behavioral Health Specialist to determine acceptance or make decisions pertaining to program.
• Facilitates meeting and education activities, coordinates logistics, scheduling and participant communications.
• Work with the external evaluator and CMO to examine the program’s interventions independently and in combination in order to evaluate program efficacy over time.

Position Requirements
MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
• Minimum Bachelor’ Degree in Nutrition or related field required
• A minimum of three year of experience in diabetes education.
• Minimum three year of prior chronic disease nutrition counseling.
• Excellent writing and communication skills.
• Certification by the National Certification Board for Diabetes Educators (CDE) preferred, but not required.  
• Other skills: Excellent oral and written communications skills required. Basic computer skills required.
