2.    Develop and write a nutritional assessment for each individual in an ICF, at least annually and individual's Day Programs as needed.
3.    Provide residential programs with seasonal menus, including modified and/or special menus for individuals residing in the program, as needed.
4.    Review and revise the nutritional service plan depending on the individual’s need or at least at least annually.
5.    Provide follow up as requested by the program nurse or physician order, regarding the  individual’s nutritional status, including but not limited to: type of diet, weight, special alerts/concerns, results of recent lab/blood test if relevant and recommendations.
6.    Review the medication record of each individual, at least annually, or whenever medication changes are ordered so as to identify potential impact on the individual’s dietary program.  Advise staff of the impact and actions to be taken to ensure the individual’s nutritional status.
7.    Consult with other clinicians, i.e., nurse, physical therapist, speech therapist, etc. regarding medical, physical or other conditions (e.g., feeding skills) which may impact on the nutritional plan for the individual.
8.    Participate in interdisciplinary team meetings as needed.
9.    Act as a resource to the nurse in regard to special dietary considerations for acute or chronic illness.
10.    Attend certification inspections and independent utilization review, as requested.
11.    Ensure correction of deficiencies in the nutrition area cited during inspections.
12.    Review the related section of the Comprehensive Functional Assessment (CFA) for each individual prior to his/her annual meeting.
13.    Provide training to staff on nutrition and other relevant topics.
14.    As needed, monitor the individuals’ weight.  In cooperation with appropriate clinicians and staff, ensure the implementation of a program designed for weight reduction/increase/ maintenance.
