Job Description
·       Evaluate and assess the nutritional status of all patients in the Dialysis Unit.
·       Recommend renal diet to responsible provider.
·       Develops and implements an individualized plan of care for each patient.
·       Document nutritional assessment, nutrition intervention, nutritional care activities and subsequent follow up in patient's medical records.
·       Reassess the nutrition care process and implement changes as indicated.
·       Communicates relevant information through interdisciplinary meetings verbally and documenting in patient's medical record.
·       Complete monthly nutritional analysis sheet on each patient which includes pre and post dialysis blood chemistries, dry weight, weight gain from fluids and pre/post dialysis weight.
·       Review monthly labs nutritional analysis sheet with patients and provide copy to patients.
·       Attend interdisciplinary conferences/meetings involving patient care, PI/QI meetings and necessary intervention.
·       Attend departmental, interdepartmental, in-service and continuing education programs.
·       Participate in the development of the interdisciplinary care plan.
·       Demonstrates accountability for the proper use of the patients protected health information.
·       Serves as a resource to the staff on nutrition issues.
·       Assists in achieving compliance with all Federal, State and Local Dialysis Regulations including Joint Commission Accreditation of Health Care Organizations (JCAHO) standards.
·       Supports and promotes the organizations infection control, safety, risk management and customer/guests relation programs.
·       Instructs and provide current educational material to patients on prescribed patient diet.

Ellenville Regional Hospital is a 25-bed Critical Access Hospital nestled in the Hudson Valley. We provide a wide range of inpatient and out-patient services to those members of our local community, and to those who visit the beauty of the area.

We are seeking a Registered Dietitian on a part time basis to be responsible for the nutritional management of our patients. Duties include evaluating and/or supervising the evaluation of the nutritional status of the patients and determine if the current diet order is appropriate, providing nutritional education for inpatients requiring therapeutic modifications to the diet, and providing nutritional education counseling for outpatients and community education programs.
Candidates must have a Bachelor's Degree as a Registered Dietitian with a NYS RD license, and one year of RD experience.

Dietitian/Nutritionist, Stonybrook NY
Required Qualifications: (As Evidenced by an attached Resume)
Bachelor's degree in Nutrition/Dietetics. One (1) year of full-time related experience providing nutritional assessments. Completion of Dietician internship approved by the American Dietetic Association. Ability to communicate effectively (verbal and written) with patients, families and the public.

Preferred Qualification:
Master's degree. Registered Dietician. Additional years of experience providing nutritional assessments and education (individual and group) within a clinical setting. Experience conducting field work and/or home visits. Knowledge of HIV disease and experience working with HIV+ individuals and diverse populations. Able to communicate (verbal and written) in Spanish.

Brief Description of Duties:
The Division of Pediatric Infectious Disease is looking for a Dietician/Nutritionist who will be responsible for providing comprehensive nutritional assessments for HIV+ patients during day and evening clinics both on-site and off-site. The incumbent will also provide extensive education regarding nutrition and HIV during individual and/or group sessions and will work with patients who are in need of emergency food assistance in order to meet their medical nutritional needs and prepare a list of essential food items that will be ordered and delivered to the patient's home.

Provide comprehensive nutritional assessments for HIV+ patients.
Conduct individual and/or group nutritional education sessions for patients.
Work with patients to identify and order food to meet their medical nutritional needs.
Keep written and computerized records for all work performed and prepare monthly activity reports to measure progress towards accomplishment of program goals.
Perform additional services at the request of the Division Chief.
Special Notes: Must be able to travel to clinics within Suffolk County. May be required to work evening hours in clinics.
