Overview: How Do I Apply?
SMDEP program sites are completely autonomous in making admissions decisions. The SMDEP National Program Office (NPO) has established
admissions guidelines regarding acceptance procedures for sites and applicants. SMDEP program sites are expected to follow these guidelines.
To apply to SMDEP, follow these steps:
Step 1: Eligibility Requirements

Step 2: Prepare Application Materials

Register with AAMC/SMDEP to obtain an identification number, username, and password.

Request an official transcript from every U.S. or Canadian post-secondary institution you have attended. The institution registrar’s office MUST mail your transcript to the SMDEP National Program Office. See
the
SMDEP Transcript Form for the mailing address, and please view the
transcript guidelines.

Request letters of recommendation. Use the
SMDEP Recommendation Form. Recommendations can be faxed to 1-866-304-7637 or mailed to the SMDEP National Program Office. Please view the
letters of recommendation guidelines.
PLEASE NOTE: To give the program sites adequate time to view all applications and make decisions, all letters of recommendation and
transcripts MUST be postmarked by Sunday, March 1, 2016. Application forms and materials can be accessed within the online application and in the
application materials section.
Step 3: Submit Application

Complete the
SMDEP Web Application—Once successfully submitted, your information is immediately available to SMDEP.
Step 4: Apply for the SMDEP Travel Scholarship

Fill out the
SMDEP Travel Scholarship Application Form
