POSITION DESCRIPTION
The Medtronic Clinical Specialist (MCS) is either a current clinical expert or “clinical expert in training” relative to Medtronic therapies (CareLink, iPro, Artificial Pancreas Technology, Insulin Forward, etc.). The MCS will be responsible for direct Medtronic therapy education and clinical protocols related to patients, health care professionals (HCP), and HCP practice staff in limited focus accounts. The MCS will gather information and knowledge through his/her work with the HCP and account healthcare team, to identify best practices from a clinical and process standpoint. 

The MCS shall not engage in any activities or conduct at any time that assists the HCP account generally on processes or administrative burdens, replaces normal HCP staff responsibilities, manages individual patient diabetes care, provides licensed health care services, or any other work that is not directly tied to the clinical support of Medtronic products and therapies. POSITION RESPONSIBILITIES
• Evaluates and tests infrastructure and protocols in limited focus accounts 
• Maintain and refine best practice protocols for training and therapy on-boarding process for both patient and HCPs. 
• Provide consultation and education to patients currently on or considering Medtronic therapies, to offer helpful information and answer questions. 
• For patients on Medtronic products, make clinical adjustments when requested by patient or HCP staff on patient’s Medtronic products only in specific accordance with existing HCP orders. 
• Assist HCP in assessing and managing patient therapy once the initiation process begins as needed to ensure a positive outcome, including troubleshooting clinical and technical issues regarding Medtronic products and therapies. 
• Deliver efficient use of Medtronic technology and services in HCP account including iPro, CareLink, pumps and sensors, and necessary Medtronic documentation that will be part of Medtronic’s patient records. Assist HCP account identify and implement more efficient protocols tied to Medtronic products and order processes. 
• Facilitate collection of all intake documents from patients, insurance, and HCPs needed for Medtronic patient records and supply Medtronic therapies and services efficiently. 
• Train patients and HCPs on Medtronic Diabetes products, operations, and therapy including how to do adjust insulin regimens for CSII using standard Blood Glucose (BG) readings and sensor technology.

Summary of Responsibilities
To provide leadership and support to nutrition services. Develop systems that ensure compliance with Head Start performance standards, NY State sanitation codes and CACFP regulations. Monitor nutrition health concerns and provide education and training materials on nutrition to parents. Provide direct oversight of comprehensive nutrition services and education that affects the nutrition development of children and families.

Qualifications/Education and Experience:
· Bachelors Degree in Food Management or related area
· Registered Dietician, current ADA Registration (American Dietetic Association) or a Master’s degree (MPH OR MS) from an approved program in public health nutrition.
· 3-5 years experience in nutrition administration and supervision.
· Must have ability to work independently

Job Summary:

Provides medical nutrition therapy to long-term care/rehab residents. Participates in collaboration with other disciplines through patient care meetings and MDS regardingpatient care.
Duties and Responsibilities:
The following functions are considered essential to this position:
-          Reviews medical record of each resident and meets with resident and/or family to obtain and evaluate diet history information of each resident.
-          Collects data and completes nutritional assessments (Initial/Annual/Significant change of status- long form assessment) and care plans for each resident including nutritional needs within the guidelines established by the Registered Dietitian.
          Reviews and co-signs initial, readmission, annual and significant change assessments of diet technician.
-          Counsels and educates resident and/or family in food selection and diet restrictions to meet nutritional needs. Documents counseling in medical record.
-          Attends Resident Care Conferences as scheduled and integrates and/or updates individual residents total plan of care. (Note these meeting are two to three days a week from 1-3:30pm) 
          Attends weekly wound, weight and fluid restriction meetings. Reviewing plans of care, approaches and interventions of affected residents.
-          Reviews and updates nourishments to meet the nutritional needs of each resident; updates and adjusts nourishments as needed.
-          Processes diet orders and changes in diet and inputs changes in computer as necessary. Updates resident profiles.
-          Charts monthly on residents at nutritional watch- Stage III, IV, DTI and unstageable wounds, tube feedings, dialysis, weight loss, decrease in intake and other residents as deemed necessary.
-          Charts quarterly on residents per schedule and charts on residents with changes in nutritional status, physical changes affecting nutrition ect, as needed.
-          Completes MDS’s per established procedures and signs signature sheets.
-          Completes CAA’s as appropriate (Therapeutic, tube feeding and dehydration)
-          Charts on lab results as appropriate (Note labs are received daily)
-          Completes audits as needed
-          Makes meal rounds to monitor residents’ intake, acceptance of meal supplements and need for assistance at mealtimes as needed.
-          Makes referrals to SLP, OT, dentist and restorative program as needed for benefit of resident.
-          Completes intake studies as needed.
-          Performs nutrient analysis on regular and therapeutic extensions.
-          Completes admission/readmission notes and notes for problems with individual residents within 48 hours of admission/readmission or notification. (Ex. Weight loss, unusual decreased intake, wounds ect.)
-          Assists Food Service Director in design and review of regular and therapeutic diets.
-          Attends Food Counsel meetings with residents.
-          Completes Guardian Angel Program Rounds- visiting and asking questions to 16 assigned residents and attending monthly meetings.
-          Completes Abaquis assignments - Family interviews and completes accompanying paper work. New assignments given monthly.
-          Other duties as assigned by Administrator such as informing residents and family of Evercare program.
-          Performs a variety of related duties as assigned for the benefit and success of the department.
-          Attends continuing education. (Must complete 75 credits every 5 years) 

Job Requirements 
Qualifications:
Must possess a Bachelor of Science from an accredited program in Nutrition and Dietetics. Must be registered by the Academy of Nutrition and Dietetics, or registration eligible. Must be able to interact with residents/staff effectively. Must possess computer skills.
