Brief Description of Duties:

The incumbent will also provide extensive education regarding nutrition and HIV during individual and/or group sessions and will work with patients who are in need of emergency food assistance in order to meet their medical nutritional needs and prepare a list of essential food items that will be ordered and delivered to the patient's home.

For the past 24 months, St. James Mercy Hospital has been engaged in a robust strategic visioning process with community stakeholders in alignment with New York State's healthcare reform agenda. Now on a fast-track growth trajectory, SJMH is working with the University of Rochester Medical Center (URMC) to develop a long-term plan for providing regional healthcare services under the auspices of UR Medicine.  Job Summary:
Responsible for the overall nutritional well-being of all chronic dialysis patients. Maintains the quality of total patient care so that the department meets all the requirements of the Code of the State Department of Health, JC and the ESRD Network. Responsibilities:
Works with Interdisciplinary team to maintain or enhance the nutritional status of ESRD the patients by assessing the nutritional needs, planning, implementing and evaluating nutritional care in correlation with health care plans.
Interviews patients and family members to obtain a nutritional history, determine current eating patterns, individual food habits, food allergies, and most pressing nutritional needs.
Recommends to the Nephrologist appropriate diet prescriptions, based on diet history, lab data, physical findings and clinical symptoms.
Participates in pre-admission ethics conferences, to assist in the decision making process of preferred modality, and provide patient education.
Instructs patients and families in therapeutic diet, and nutritional principles, adapting instructions to accommodate individual economic, educational and life style requirements. Provides ongoing support in maintaining therapeutic diets.
Routinely reviews and interprets patients’ lab data, medical status, and kinetic modeling information. Discusses this information with patients, and recommends change in their eating patterns, based on the results of this information.
Documents appropriate information in the patients chart, including nutritional assessments, nutrition care plan, nutrition educational, review and evaluation of patients’ progress.
Attends and participates in Interdisciplinary Patient Care Conferences. Maintains the Interdisciplinary Care Plans.
Regularly reviews and upgrades patient nutritional education materials to assist in instructions of therapeutic diets. Maintains a file of menus and recipes suitable for use in Renal diets.
Participates in staff and student orientation. Conducts inservices to Dialysis and Nutrition staff as requested.
Reviews and may assist in development of policies and procedures that directly affects nutritional aspects of patient care.
Maintains a dialysis specific quality assurance program. Completes and reports quality assurance audits as required.
Prepares monthly statistical reports for review.
Keeps informed of current practices and trends in the field of Renal Nutrition via participation in the activities of the Council of Renal Nutrition, attendance at Renal Specific Seminars, and review of journal articles.
May participate in and/or develop clinical investigations that have a nutritional component.

Job Specifications:
Bachelor Degree with a major in Foods and Nutrition (Completion of dietetic internship, cooperative undergraduate program, or Masters program).
Registered Dietician, Member of the American Dietetic Association. Preferred member of Council of Renal Nutrition.
Minimum of one year experience in clinical nutrition. Preferred experience in Renal Nutrition.

Aptitudes:
Verbal ability required to understand medical and scientific terminology in order to communicate with professional staff regarding technical aspects of the department (Ability to understand policies, principles and procedures of dietic and hospital administration).
Ability to communicate with personnel at all levels
