New York City Department of Health & Mental Hygiene Data Enterer
GENERAL RESPONSIBILITIES: The Healthy Eating Initiative within the New York City Department of Health and Mental Hygiene’s Bureau of Chronic Disease Prevention and Tobacco Control is seeking a Data Enterer to support the evaluation efforts of seasonal nutrition education programming that takes place at farmers’ markets and at child care centers across NYC. The Data Enterer will be responsible for ongoing management of data obtained in the field from program participants. This paid position is part-time and seasonal. The selected candidate(s) will be required to commit to two days per week from mid-July through mid-December 2016. 

DUTIES: Duties will include, but are not limited to: • Enter data from nutrition workshop participant surveys into MS Access and MS Excel databases • Enter hours worked for consultants into MS Excel tracking sheet from timesheets and ensure correct formulas are being maintained in database • Organize, maintain and store surveys • Identify and correct data entry errors as noted by program staff • Meet monthly reporting deadlines

As a Staff Dietitian you will provide nutritional care and guidance to WIC program participants. Perform related duties supporting program operations.  Responsibilities Include:

Provides nutritional care and guidance to WIC program participants:
Interviews patients to determine nutritional status and establish priorities. Obtains patient anthropometrics, hematology results, breastfeeding status, and related information.
Reviews patient records for timely immunization of infants and children.

Contributes to initial and ongoing comprehensive care planning and attends comprehensive care planning meetings, morning rounds when on site.
Develops and facilitates group sessions in consultation with interdisciplinary team to address nutrition, menu planning, cooking, and current trends in diet therapy to clients in program.
Provides in service education to the program staff and volunteers.
Acts as liaison to external care providers in regard to possible interaction of medical condition, drug treatment, and nutritional status.
Maintains client caseload for purpose of coordinating care both within program and with outside case management providers.
Supports and participates in advocacy efforts to end the twin crises of homelessness and AIDS.

Education/Skills Requirements
The candidate must be registered as a Dietitian from a school accredited by the American Dietetic Association with a Master's degree from an accredited college or university. Two years clinical experience in a health setting preferred. Experience in working with HIV/AIDS population desirable.

General Duties :
The Registered Dietitian is responsible for the development of all personalized menus for the residential individuals they are assigned to. He or she will conduct routine observation of food preparation at the assigned residences and guarantees that all meals are nutritionally sound, varied and palatable. Menus are updated twice per year for Spring/Summer and Fall/Winter. Dietitian works with each residence to fine tune each menu to include likes and cultural preferences.

Upon review of each medical chart inclusive of weight charts and lab work at least semiannually, the Dietitian is responsible for recommendation of diet needs of all assigned individuals to the primary physician.
Work very closely with the Speech Pathologists and Nurse to ensure modified diets are prescribed as part of the total health or nursing care plan.
He/she will prepare at minimum annual nutritional assessments for assigned individuals which are made part of permanent medical record. These assessments are presented at annual ISP meetings where their presence is expected.
The Dietitian will analyze prescribed diets, set goals and supervise that the diet plan is carried out by those preparing the food and provide training as needed.
He/she reviews nutritional plan each month and notes progress or problems.
Maintains a file system containing pertinent data (data/age/name/weight/etc.)
Maintains nutrition notes in medical charts when appropriate.
