Position Reports to: Health Bucks Project Manager, Bureau of Chronic Disease Prevention and Tobacco Control

The Fund for Public Health in New York, Inc. (FPHNY) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers. To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNY implements programs to address pressing public health needs, fosters private sector support to enhance health and health care, and helps educate the public regarding the protection of individual, family and community health.

 The Fund for Public Health in New York seeks a Fruit and Vegetable Incentive Coordinator, funded through a federal grant to help expand the Health Bucks program. Health Bucks – $2 coupons redeemable for fruits and vegetables at all New York City farmers’ markets – is the nation’s largest city-operated farmers’ market incentive program and is a practice-tested intervention for addressing obesity in low-income communities. Health Bucks are distributed as a SNAP incentive at more than 120 farmers’ markets and through nutrition programming at more than 350 community-based organizations each summer and fall.

 This position will be housed within the Bureau of Chronic Disease Prevention and Tobacco Control, which is in the DOHMH Division of Prevention and Primary Care. The bureau spearheads programs and initiatives to reduce the burden of chronic diseases and to address the underlying risk factors that lead to them, such as poor nutrition, physical inactivity and tobacco use. The Bureau promotes healthful environment and systems changes in the food and physical environment and develops programs that promote physical activity, healthy eating and smoking cessation. The Bureau also develops innovative strategies to convey critical health messages to the public. This position is funded through March 31, 2020; position is contingent upon availability of federal funding and successful annual reapplication for funding. Award expected in April 2016.

POSITION OVERVIEW:
The Fruit and Vegetable Incentive Coordinator will be responsible for expanding the Health Bucks program to increase the affordability of fresh, locally-grown fruits and vegetables for low-income New Yorkers.

RESPONSIBILITIES:
·         Expand and coordinate distribution and tracking of Health Bucks coupons to farmers’ markets to include year round distribution.
·         Support farmers’ market operators in efficient and accurate distribution of coupons ensuring they are reaching SNAP participants.
·         Coordinate implementation of a pharmacy-based fruit and vegetable prescription pilot program for SNAP participants.
·         Coordinate implementation of a fruit and vegetable incentive pilot program for SNAP participants at healthy retail stores.
·         Support evaluation efforts related to Health Bucks and fruit and vegetable incentives.
·         Coordinate with the Fund for Public Health regarding grant requirements including reporting, spending, setting up and monitoring contracts and ensuring program is meeting grant deliverables.
·         Coordinate and assist with program evaluation.
·         Assist with development of marketing campaign to promote the program.
·         Serve as the representative of DOHMH in meetings, give presentations, and develop a wide range of written materials.
·         Take responsibility for day-to-day operations of the project including tracking progress and spending.
·         Perform other duties as assigned, including data entry, coordinating meetings, and supervising interns

 QUALIFICATIONS:
·         Master’s degree with a specialization in an appropriate field of physical, biological, environmental or social science, nutrition or public health
·         3-5 years of full-time professional, technical or evaluation experience in one or more of the following fields: program coordination, community outreach, and/or administration
·         Willingness to travel throughout the 5 boroughs of NYC and occasionally work on weekends as needed

 PREFERRED SKILLS:
·         Experience with coordinating farmers market-based nutrition programming, working in multi-cultural settings with diverse populations
·         Knowledgeable about community food security and health equity issues in New York City

Clinical Dietitian/Manager, Nathan Littauer Hospital, Gloversville, NY

Sodexo is seeking a Registered Dietitian for a Clinical Nutrition Manager position at Nathan Littauer Hospital and Nursing Home.  Located ~1 hour northwest of Albany, this community hospital has acute care, long term care and an outpatient clinic.

Reporting to the Food Service General Manager, the Clinical Nutrition Manager will:

Be a Registered Dietitian with management or supervisory experience.
Provide nutritional care to the long term care residents and backfill acute care and outpatient as needed.
Manage the nutrition program; supervise and develop a team of 2 RD’s and a Dietetic Technician.
Assist in managing the room service program and supervising the patient ambassadors.
Demonstrate skills in accreditation and regulatory readiness and review; and in establishing and maintaining standards of care.
Work every third weekend, primarily providing nutritional care to the acute care patients.
Interact with medical staff and nursing on a regular basis; represent the department on hospital-wide medical staff committees.
Demonstrate strong verbal and written communication skills as well as great time management and organizational skills.
