Job Qualifications:
The candidate must possess excellent interpersonal and communication skills, as the position involves extensive interaction with patients, physicians, and clinical staff. He/she should be capable of performing both independently and as part of a team, demonstrating the ability to multi-task and take initiative. The individual will work closely with two other metabolic dieticians in addition to a multidisciplinary clinical team. Clinical experience with inborn errors of metabolism is preferred, pediatric nutrition experience is required. This is not an entry level position, three or more years as a clinical dietician is a must.  Masters degree in science, registered dietician status and CDN is required. Command with the Spanish language is a plus.

Principal Duties and Responsibilities:
Provides nutritional care and support to in-patients and outpatients according to diagnosis specific guidelines and under the immediate supervision of the clinical nutrition director.
Creation of disease-specific dietary management plans for infants, children, adolescents, and adults with inborn errors of metabolism in the outpatient and inpatient settings.
Performs ongoing nutritional assessments on patients to determine nutritional status, individualizes, monitors and updates nutritional care plans based on laboratory data and clinical findings to optimize nutritional intake and maintain appropriate growth. Consults with physician whenever necessary.
Interview patients/family members to obtain information regarding the patient’s nutritional status, food habits and preferences to assist with individualizing patients nutrition care plan.
Discusses dietetic goals and barriers with patients and their families and counsels patients and /or their families individually or in groups, on ways to meet dietary adherence in a long range.
Works closely with other metabolic dieticians, attending physicians, nurse practitioner, clinical coordinator, residents, social worker. Participates in patient care conferences and results meeting.
Participates in the development and revision of nutritional educational materials to improve patient quality of care specify for diagnosis, age and literacy level.
Maintains professional growth and development through continuing education, professional meetings and conferences to keep informed of current practices and trends in the fields of dietetics and nutrition.  Applies current practices to nutritional care.
Demonstrates organization in performing several tasks simultaneously. Shows willingness to move from one task to another as work requires without being asked.
Extends nutritional care to patients reporting laboratory results within 24 hrs of being posted via phone and/or email with treatment plan and recommendations.
Ensure customer satisfaction inclusive of replying to all emails and/or phone calls placed by patients and/or family members regarding nutritional care on a daily basis.
Prepares all prescriptions needed for metabolic formulas.
Documents in the medical record all ongoing nutrition interventions (one on one, email or phone).
Receives interns from the internship/preceptorship of hospital program

Also a minimum of four (4) years’ experience working in the field of Human Services, Education, Health & Nutrition or Community Organization of which at least two (2) years include management experience.
Working knowledge of CACFP. An Associate’s degree in a related field AND six (6) years’ experience working in the field of Human Services or Community Organization may be accepted in lieu of a Bachelor’s degree.
Strong interpersonal & communication skills. Solid PC skills including database entry, word processing, and spreadsheet skills are required. Bilingual or multilingual abilities a plus. Possession of a valid driver license is required.
Knowledge of Early Childhood Development, practices and theories, Head Start Performance standards, Day Care regulations, including state and local regulations desired. Total Quality management/continuous improvement training and experience preferred.

The program, known as La Nueva Esperanza, provides effective comprehensive food and nutrition services to HIV positive Latino and African American residents living in Brooklyn. La Nueva Esperanza program, solely funded via this Ryan White Title I contract, identifies clients who are or are at-risk of malnutrition, hunger, and are in need of food and nutrition counseling services. Eligibility requirements are addressed through an intake/assessment process. Each client’s medical history, knowledge of nutrition and HIV/AIDS, and their ability to prepare and/or access nutritious meals are assessed. Barriers that may lead and/or have led to hunger, malnutrition, poor eating habits, side effects to medication, and medical illnesses are identified and client-specific goals are defined. To achieve these goals, La Nueva Esperanza contract-funded staff provide nutrition counseling and education, congregate and home delivered meals, and pantry bag distribution to assist clients understand the importance of nutrition, eliminate or reduce barriers that contribute to hunger, malnutrition, non-adherence to medication, and identify strategies that promote healthy eating habits. Services are provided in a culturally competent manner, acknowledging our clients’ diverse beliefs, values, and backgrounds.

Client enrollment occurs via the program’s recruitment efforts. Upon enrollment, a contract-funded staff member conducts an intake/assessment. Each client signs a Service Plan outlining individualized short and long-term goals identified via the assessment process. Food services – congregate meals, home delivered meals, pantry bag distribution (FRIDAY's ONLY)– are available five days a weeks with most clients accessing about six service encounters per month. Nutrition counseling and/or nutrition group education are provided monthly to assist the clients achieve their Service Plan goals. Reassessment and Service Plan development occur on a semi-annual basis.
