Cornell Cooperative Extension in NYC has immediate openings for bi-lingual nutrition community educators (Mandarin/English or Korean/English).  Community educators work as part of a team and assist in planning, implementing, documenting and evaluating nutrition /health education programs / initiatives for adults and youth in targeted communities in the NYC metropolitan area where large numbers of Asian families reside (Brooklyn, Queens, and Manhattan).

Educators assist in the recruitment of agency partners, program participants; teach relevant food, nutrition, budgeting, food safety modules and facilitate skill development of participants who speak primarily Mandarin, Cantonese, Bengali and Korean. In additions, these staff members assist in recruiting and supporting volunteers; document program activities and record outcomes. Completion of Initial Nutritional and Hydration Care plans.
Assess long term resident annually.
Attend meetings which pertain to the dietary discipline as available.
Completion of MDS assessments.
Monitor weight status of all residents.
Complete episodic nutrition notes (i.e. weight changes, antibiotic usage, changes in intake)
Monitor Lab values as drawn, note changes as needed.
Complete Tube Feeding order form and monitor for needed changes.
Document nutritional interventions for any new skin issues.
Council residents regarding therapeutic diets and nutritional goals.
Evaluation of menu’s for nutritional adequacy.
Any other duties as assigned by Administration

MAJOR PURPOSE: To provide nutritional care and education to the residents and/or families, and participates as a decision making member of the Care Plan Team. Supervises the work of Clinical Dietician II.

QUALIFICATIONS:

PRIMARY FUNCTIONS:
Integrate the food and nutrition aspects of resident care through communication with the medical and nursing staff.
Follows OBRA regulation for completing the appropriate section of the Minimum Data Set (MDS).
Contributes pertinent information to resident care plans and records appropriate progress notes in the resident's medical record.
Provides nutritional counseling to residents and their families.
