Under the direction of the Diabetes Program Manager, the Diabetes Care Manager(DCM): Certified Clinically Advanced Nutrition Coordinator acts as an integral part of the health care team onsite at Mount Sinai physician network practice sites. The DCM: Certified Clinically Advanced Nutrition Coordinator formulates cohesive relationships with physicians, administrators, health care professionals, and office staff at their assigned practice sites to foster the implementation and ongoing delivery of Diabetes Self Management Education/Training (DSME/T) and diabetes patient care.

Documents Clinical Effectiveness: Documents all patient encounters (face-to-face and telephone) as per departmental protocols.  Ensures that clinical process measures related to diabetes and cardiovascular disease are clearly documented.  Supports patient goal setting and documents patient's progress in achieving their goals as per protocols.  Adheres to the American Diabetes Association's guidelines for documenting patient goals, patient education, and outcomes related to patient's knowledge base, therapeutic lifestyle changes, and self care activities.
Builds Nutritional and Weight Management Plans and Educational Offerings:  Develops Mount Sinai branded culturally sensitive meal planning and weight loss collaterals that can be utilized across all patient care setting (inpatient, ambulatory care, and network physician practice sites) in support of preventing diabetes and/or weight loss for those with diabetes and or cardiovascular disease.  Builds cohesive relationships with physicians from the Department of Bariatric Surgery, Diabetes Center, and Cardiology to support the multidisciplinary approach to weight loss and healthy eating for those with diabetes, at-risk for developing diabetes, and cardiovascular disease which can be adopted at select physician network practice sites.
Supports the Development of a Medical Team for Patients: Establishes a medical team for patients with diabetes, pre-diabetes and cardiovascular disease to support the goals of care outlined by the American Diabetes Association.  Works effectively with Mount Sinai specialists (endocrinologists, cardiologists, bariatric surgeons, and vascular surgery) working onsite at network physician practice sites to ensure patients have access to specialty care to avoid adverse events.
Community Outreach and Marketing:  Represents Mount Sinai at community based Diabetes and Cardiovascular Events.  Educates the public and at risk communities in a culturally sensitive manner on optimal nutritional practice to support overall wellness.  Links patients with specialty services available at Mount Sinai to ensure access to diabetes and cardiovascular specialty care.

Supports the clinical integration of new diagnostic tools, electronic medical records, and health care technology into daily practice operations.
Supports the implementation of patient satisfaction and retention initiatives to support the ongoing growth of patients under management by the network.
Monitors practice to ensure compliance with: Mount Sinai's compliance standards, OSHA requirements, HIPAA compliance, emergency preparedness, universal precautions, JCAHO and practice certification standards, and managed care provider standards.

Qualifications:
5-10 years of clinical experience preferred, 5 of which are in the diabetes arena.  Demonstrated diabetes program implementation experience and ability to work effectively with a multi-disciplinary medical team.  Experience in working within community-based diabetes initiatives, and with busy primary and specialty care physician practices located throughout an urban and suburban territory.  Implemented diabetes disease specific care management initiatives onsite at busy physician practices and has experience working as CDE within busy physician practice locations.

The Fund for Public Health in New York, Inc. (FPHNY) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers. To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNY implements programs to address pressing public health needs, fosters private sector support to enhance health and health care, and helps educate the public regarding the protection of individual, family and community health.
The Fund for Public Health in New York seeks a Health Bucks Workshop Specialist, funded through a Partnership to Improve Community Health (PICH) grant from the Centers for Disease Control and Prevention (CDC).  This grant supports an integrated set of complementary evidence-based activities to bring about environment, program, and infrastructure changes to improve health outcomes by preventing the leading causes of death and disability across the life span for all New Yorkers, particularly those who experience disparities in health. The risk factors being addressed in this grant include addressing poor nutrition and physical inactivity as well as reducing tobacco use and exposure to secondhand smoke.

This position will be housed within the Bureau of Chronic Disease Prevention and Tobacco Control, which is in the DOHMH Division of Prevention and Primary Care. The bureau spearheads programs and initiatives to reduce the burden of chronic diseases and to address the underlying risk factors that lead to them, such as poor nutrition, physical inactivity and tobacco use. The Bureau promotes healthful environment and systems changes in the food and physical environment and develops programs that promote physical activity, healthy eating and smoking cessation. The Bureau also develops innovative strategies to convey critical health messages to the public. This position is funded through September 29, 2017.

POSITION OVERVIEW:
The Specialist will be responsible for coordinating and supporting farmers’ market-based nutrition programming led by community-based organizations and DOHMH seasonal staff.  Both programs support the expanded distribution of Health Bucks to low-income New Yorkers.

RESPONSIBILITIES:
·       Work with DOHMH staff to coordinate farmers’ market events from July- November including the hiring, training and supervising seasonal staff as well as assisting with staff meetings and providing ongoing technical assistance to maximize Health Bucks distribution
·       Communicate with the public, partners, and key stakeholders about program efforts undertaken as part of PICH
·       Revise and expand program implementation guide for community partners to conduct nutrition activities for a variety of audiences
·       Ensure program partners meet deliverables and submit paperwork in a timely fashion.
·       Submit purchase order requests for program materials and supplies
·       Conduct site visits and onsite trainings to ensure quality implementation of nutrition activities and Health Bucks distribution on weekdays and occasional weekends
·       Coordinate a process evaluation of farmers’ market-based nutrition activities with guidance from Health Department evaluators

QUALIFICATIONS:
·       Master’s degree with a specialization in an appropriate field of physical, biological, environmental or social science, nutrition or public health
·      1 year of full-time, satisfactory professional, technical or evaluation experience in one or more of the following fields: program coordination, community outreach, training development, and/or administration.

PREFERRED SKILLS:
·      Experience conducting community health workshops and/or nutrition workshops in group settings; excellent group facilitation skills
·      Ability to work collaboratively within a program coordination team
