We are looking for a Program Manager for our brand new program at Nassau University Medical Center in Long Island. In this role, you will play a key leadership role integrating Health Leads into the clinical services at NUMC; working alongside providers and coaching and developing a talented corps of college undergraduates to successfully connect patients to resources vital to their health.  


We are looking for Spanish-speaking professionals with case management background (looking for 3-5 years), especially with vulnerable populations, an interest in mentoring and managing college undergraduates as well as being part of a national organization trying to make systemic impact on the healthcare delivery system.
