Clinical Dietitian position - Full Time, Brooklyn, NY, Long Term Care


Minimum Qualifications:

• Baccalaureate Degree with major in dietetics or food and nutrition from a program                                                       approved by the Academy of Nutrition and Dietetics (formerly American Dietetic Association)

• Registration or eligibility by the Commission on Dietetic Registration (CDR) as a Registered Dietitian.

• Minimum of three years of clinical experience.

• Attends continuing education programs to keep abreast of new guidelines in the field.

• All position are subject to a background investigation.

Responsibilities:

• Performs nutrition assessments and reassessments daily as per assigned unit, admission date and time as per the current policy of the department.

• Visits residents to obtain a detailed nutrition history summary and determine their nutritional needs. Once needs are determine their nutritional needs. Once needs are determined, comprehensive assessments will be completed including recommendations that will be communicated to physicians and/or nursing staff.

• Performs and documents meal rounds to observe residents acceptance of meals. Once observed, actions to correct the residents' concerns regarding their meals should be taken which includes obtaining likes and dislikes and allergies and communicating them with the kitchen and entering them into the Geri Menu System.

• Provides nutrition education and counseling to residents (and family if appropriate) identified at nutritional risk prior to discharge. Education is to be reflective of the resident's nutritional needs and in accordance with therapeutic diet guidelines.

• Participates in discharge planning rounds, interdisciplinary team rounds, Wound Care Rounds on assigned unit to discuss the plan of care and recommendation with the interdisciplinary team.

• Reviews daily census sheets and monthly MDS report to determine which residents need to be seen.

• Documents all assessments, reassessments, likes, dislikes, allergies and all other pertinent information in medical record.

• Documents daily productivity of all patients assessed and/or visited and any patient associated meetings daily. Provide daily productivity reports to the clinical nutrition manager on a weekly basis.

• Attends and participates in department monthly meetings - may plan and conduct meetings in the Clinical Nutrition Manager's absence.

• Performs other related duties as assigned.
