Position Summary:
To provide for the nutritional care for all patients within the scope of responsibility and act as consultant to other health care professionals in determining the best nutritional care options for patients.

Key Responsibilities:
Completes Nutritional Assessments for all patients that fall within our scope of responsibility.
Provides oversight to dietetic technicians in providing patient care. Develops plan of care per clinical data, interview, diagnosis and treatment goals.

Nutrition Project Coordinator, Fund for Public Health in New York, Long Island City, NY
The Fund for Public Health in New York, Inc. PROGRAM OVERVIEW
The Healthy Eating unit at DOHMH develops new models for encouraging healthy eating through education and strategies directed at a range of venues, such as schools, hospitals, and restaurants. Key programmatic activities include developing and leading the National Salt Reduction Initiative, MenuStat, NYC Food Standards, and Good Choice.

This position will be housed within the Bureau of Chronic Disease Prevention and Tobacco Control, which is in the DOHMH's Division of Prevention and Primary Care. The Bureau spearheads programs and initiatives to reduce the burden of chronic diseases and to address the underlying risk factors that lead to them, such as poor nutrition, physical inactivity and tobacco use. The Bureau promotes healthful environment and systems changes in the food and physical environment and develops programs that promote physical activity, healthy eating and smoking cessation. The Bureau also develops innovative strategies to convey critical health messages to the public. This position is contingent upon the availability of federal funding and successful annual reapplication for continued funding.

POSITION OVERVIEW:
The Fund for Public Health seeks a Nutrition Project Coordinator, funded through the Sodium Reduction in Communities Program (SRCP) grant from the Centers for Disease Control and Prevention (CDC). The program aims to increase access to lower sodium food options and reduce sodium intake. The Nutrition Project Coordinator will be a key team member in supporting the development and execution of innovative approaches to changing the food environment.

RESPONSIBILITIES:
Identify, recruit and support technical assistance provided to a range of sites, such as City agencies, hospitals, universities, and/or community organizations, that implement the NYC Food Standards

Job DescriptionNeighborhood WIC, a program of Public Health Solutions, provides special supplemental foods, nutrition education and counseling to low-income pregnant women, infants and children up to age five who are at nutritional risk. We are seeking bilingual (English/Spanish) Nutritionist to assess the eligibility of WIC applicants, implement the nutrition education component of our program and supervise support staff (WIC clerks, Program Assistant).

Specifically, the Nutritionist will:
• Verify and document eligibility of WIC participants including participation in hematological & anthropometric activities
• Provide individual and facilitated group discussion
• Assist in developing and evaluating literature, posters, videos and DVDs
• Participate in NYSDOH WIC Quarterly Assurance Evaluation to ensure compliance with WIC's regulations and enhance efficiency in core services offered
• Provide accurate documentation for measuring changes in participants nutritional status
• Maintain internal (Public Health Solutions programs that are pertinent to WIC participants) and external relationships (Community based organizations and providers)

Qualifications:
• A Bachelor's degree from accredited college with a major in foods and nutrition, including at least 18 semester credits in nutrition
• Proficient computer skills, WICSIS documentation - determine risk factors
• Bilingual (English/Spanish)
• Must have exemplary customer service skills

As part of an interdisciplinary team within our Medical Services division, the professional we select will provide patients with counseling and nutritional assessments, and calculate prescription diets. PURPOSE AND SCOPE:
Supports FMCNA’s mission, vision, core values and customer service philosophy.  Adheres to the FMCNA Compliance Program, including following all regulatory and FMS policy requirements.
Provides nutritional services for the facility’s in-center and home patients as applicable in order to maximize the patient’s nutritional status and improve clinical outcomes.  Functions as a member of the interdisciplinary healthcare team, supporting the FMCNA commitment to the Quality Enhancement Program (QEP) and Quality Assessment and Performance Improvement (QAI) activities.  Actively participates in process improvement activities that enhance the likelihood that patients will achieve their individualized patient-specific goals as determined by the patient’s physician.

DUTIES / ACTIVITIES:
CUSTOMER SERVICE:
Responsible for driving the FMS culture through values and customer service standards.
Accountable for outstanding customer service to all external and internal customers.
Develops and maintains effective relationships through effective and timely communication.
Takes initiative and action to respond, resolve and follow up regarding customer service issues with all customers in a timely manner.

PRINCIPAL RESPONSIBILITIES AND DUTIES:
Assesses patient’s knowledge of diet and kidney disease and provides education appropriate to patient’s learning needs regarding the relationship of diet and kidney disease and the impact on the treatment process.
Provides patient specific detailed education to patient/family/caregiver regarding nutritional status, adequacy measures and results, fluid status, and all nutrition related lab parameters pertaining to their disease process.
Completes nutritional assessment and participates in the interdisciplinary comprehensive assessments.
Participates with the interdisciplinary team in the development of a written, individualized comprehensive plan of care that specifies the services necessary to address the patient’s needs, in accordance with established Federal and, where applicable, state licensure guidelines.
Calculates diet prescription according to standard of practice.
Completes progress notes on all chronic dialysis patients according to established FMCNA guidelines.  Maintains complete and accurate documentation of all appropriate information in medical record according to established FMCNA policies and Federal and state regulations.
Provides patient with ongoing nutrition assessment and outcome-oriented nutrition counseling necessary to assist patient in achieving and sustaining an effective nutritional status.
Identifies malnourished patients as well as patients at risk for malnutrition and works collaboratively with interdisciplinary team to identify appropriate interventions, resources or solutions.  Reviews each patient’s albumin level and body weight at least monthly. Implements plan of care with involvement of patient/family/caregiver.
Monitors adherence and response to nutrition therapy, adjusts therapy as needed to achieve and sustain an effective nutritional status, documents why the patient was unable to achieve the goals and implements the plan of care to address issues impacting the patient’s ability to achieve the goals.
Collaborates with the interdisciplinary team on Urea Kinetic Modeling recommendations to achieve the physician’s prescription of Kt/V.
Collaborates with the interdisciplinary team on management and recommendations for mineral metabolism and renal bone disease, anemia and iron status, nutritional status, psychosocial status, vascular access, modality, rehabilitation status and sodium/volume control.
Adheres to FMCNA algorithm and medical record policies.
Reports on nutrition QAI results and participates in the interdisciplinary QAI program.
Collaborates and coordinates with team members in provision of Pre-ESRD education as appropriate.
When appropriate, works with Case Manager to coordinate plan of care for patient education and identifies candidates to refer to physician for nutritional supplements, as per disease management agreements.
Reviews nutrition related lab results:
-  Counsels patient and/or caregiver and formulates appropriate action.
-  Recommends treatment changes to the interdisciplinary team as appropriate.
-  Communicates with physician and/or facility staff regarding lab results as appropriate.
-  Communicates lab results and recommended treatment changes to providers of patient care in nursing facilities (i.e. hospitals, short term and long term care facilities).

Participates in pertinent staff meetings (i.e. general staff, quality improvement), Care plan meetings, and others as applicable.
Consults with FMS Corporate Dietitian regarding nutrition concerns as needed.
Maintains and improves knowledge and skills for a competent and innovative practice.
Maintains dietetic registration and continuing education hours as specified by American Dietetic Association and state licensure regulations where applicable.
Collaborates with interdisciplinary team, ensuring that all work areas are safe and clean.
Other nutrition management duties as assigned.

* Plans, directs, implements and evaluates the Nutrition Education component of the WIC Program.

* Prepares confidential records and cooperate fully with State Investigative Services staff, as well as other authorized persons, agencies and entities identified in NYS WIC contracts.

* Four (4) years experience as a Nutritionist in a clinical or community setting, of which at least two (2) must be in a WIC Clinic. Supervisory experience highly desirable
