Community Pediatric Programs (CPP), a partnership of Children’s Health Fund and the Children’s Hospital at Montefiore, is a Federally Qualified Health Center that provides comprehensive, patient-centered health care to medically underserved children and families. CPP’s two community health centers in the South Bronx, and multi-site mobile medical program serving family homeless shelters throughout New York City, reach over 11,000 patients each year. Our programs provide high-quality primary care, mental health, case management, dental, nutrition, health education, and other services. Our mission—to close the gap in health disparities through the use of best practices and the development and evaluation of innovative models of care.

Community Pediatric Programs is seeking to hire a dynamic Health Educator to provide individual and group health education using theory- and evidence-based approaches. As part of a multi-disciplinary, mission-driven team of health professionals, the Health Educator’s main focus is CPP’s Childhood Asthma Initiative, working with children and their families to address barriers to asthma management, improve outcomes, and enhance quality of life. Other focus areas include preventive health care, chronic disease management, and wellness. The Health Educator will develop and review health education materials, ensuring appropriate reading level and cultural sensitivity, and participate in community outreach and events. The Health Educator will also contribute to related program evaluation and quality improvement efforts, including data collection, management, and analysis to assess the impact of health education activities and interventions.

Essential Functions:
•         Assess educational needs of target populations;
•         Conduct individual and group health education sessions;
•         Develop and review health education curricula and materials;
•         Coordinate clinic- and community-based health education activities;
•         Compile compendium of evidence-based programs and materials for key focus areas;
•         Contribute to development of health education activities and interventions;
•         Collect, manage, and analyze data for program evaluation, quality improvement, and reports; and
•         Cultivate and maintain community partnerships related to health education outreach and events

Qualifications:
•         Masters in Public Health (MPH, MSPH) or Masters in Health Education or Education (MS, MEd) required.
•         Certification as Health Education Specialist (CHES or MCHES) preferred and/or Asthma Educator Certification (AE-C) strongly preferred.
•         Exceptional communication, interpersonal, and presentation skills.
•         Computer proficiency, including Microsoft Office (Word, Excel, PowerPoint, Access); working knowledge of SPSS a plus.
•         Background in program planning/evaluation and data management/analysis preferred.
•         Bilingual English/Spanish strongly preferred.
