Live Healthy! Nutrition Education Instructor - New York Common Pantry

The New York Common Pantry, the largest community based emergency feeding organization in New York City, is seeking a full-time Bilingual Nutrition Education Instructor for its Live Healthy! program. As a part of the East Smart NY Program, Live Healthy! offers hands-on, interactive nutrition education workshops to the children and adults of New York City aimed to improve the overall quality of life. A strong emphasis is placed on the nutritional value of fresh fruits and vegetables and their impact on healthy living and longevity. This job is a full-time, exempt position and requires weekend work. Work schedule is set according to classes offered offsite. Extensive travel on public transit is required.

Job Responsibilities

• Provide 40 off-site and on-site activities including nutrition education classes, cooking demonstrations and active living activities to program sites within Upper Manhattan and the Bronx.

• Engage participants in environmental activities focused on healthy eating and overall wellness.

• Lead all aspects of preparation, implementation, instruction and take-down for all activities.

• Provide task supervision of assigned Program Aide.

• Organize food for each nutrition workshops; assist with food procurement and recipe preparation and demonstration.

• Engage in communication with community partners about class scheduling and organization.

• Ensure that there is adequate supply and materials for each activity.

• Transport equipment and supplies to and from activities. Lifting equipment in excess of 50 lbs.

• Ensure that all attendance records and reporting methods are completed accurately

• Enter client activity and research data into agency authorized databases.

• Lead agency run field trips to Farmer's markets and Upstate farms.

• Perform other duties as assigned by the Nutrition Education Program Manager.

Qualifications: Associate’s Degree in Dietetics/Nutrition Education and experience in nutrition education, exercise training or other health related work. Required. A Bachelor’s degree is preferred. Strong personal interest in cooking, active and healthy living. Strong Public Speaking skills and experience. Strong clerical and organizational skills. Must be detail oriented and able to multi-task. Must be comfortable working with diverse populations and age ranges. Must be bilingual, either English/Spanish or English/Mandarin/Cantonese. Must be able to lift 50lbs+. Proficiency in Microsoft Word, Excel and PowerPoint. Knowledge of databse programs a plus.

Compensation: Commensurate with experience.

POSITION SUMMARY
The Center to Advance Palliative Care (CAPC) is a national, non-profit organization that successfully uses data-driven and analytical principles to promote social and policy change in the health care field. CAPC is dedicated to increasing the availability of quality palliative care services in hospitals and other health care settings for people with serious illnesses. CAPC provides health professionals with the tools, training and technical assistance necessary to start and sustain successful palliative care programs in hospitals and other health care settings. CAPC also hosts the National Palliative Care Registry, the only repository for data on hospital-based palliative care programs in the U.S.
CAPC is seeking a full-time research associate with a background in public health research or health services research. The ideal candidate will have a minimum of three years of experience in a nonprofit environment managing diverse projects encompassing the design and implementation of program evaluations, development of process and outcome metrics, design and management of survey research projects, and the use of data visualization tools for effective and strategic communications.
Responsibilities:
§ Manage all aspects of the National Palliative Care Registry, including database management, oversight of annual survey, data collection, cleaning and analysis, and writing summary reports; work with team to develop ongoing strategies for registry improvement and expansion
§ Design web-based reporting tools using benchmarking data collected in the Registry
§ Work with VP, Research, field experts and CAPC consultants to launch a national palliative care registry for the non-hospital health care setting (e.g., nursing homes, home care, cancer centers, office practices, etc.)
§ Analyze national datasets to monitor the prevalence and distribution of hospital palliative care programs
§ Respond to requests for national health care statistics from CAPC leadership and consultants, its constituencies, and the press
§ Work with the communications department to interpret research findings for lay and professional audiences and to conduct market research for CAPC’s products
§ Design and implement evaluations of CAPC’s products and initiatives


§ Summarize and interpret evaluation results; make recommendations for program modification, as appropriate
§ Manage grants; write progress reports for projects funded by private foundations
§ Assist in writing manuscripts for publication

Qualifications:
§ Five years of related experience
§ Advanced knowledge of quantitative and qualitative research methods
§ Strong database management skills
§ Experience with data collection and survey methods
§ Excellent data analysis skills and facility with statistical software, preferably SPSS or SAS
§ Strong written and oral communication skills
§ Must be highly organized and detail oriented
§ Project management experience
§ Able to work well in a small group environment
§ Team player who takes initiative and works within department priorities
§ Demonstrated interest in palliative care, aging, and/or health care services a plus

Education:
Master’s degree in public health or a related field.
Travel:

Key Duties and Responsibilities 
Deliver strong operational performance by executing against ARAMARK Healthcare and regulatory agency standards and programs, continually monitoring operations and completing assessments and necessary action plans to provide optimal nutrition care and meal service to patients/residents. Drive patient satisfaction and maintain client relationships through rounding. Ensure compliance to food safety, sanitation and overall workplace safety standards. Supervise, staff, train, conduct applicable rounding and manage the performance of the registered dietitians and dietetic technicians regarding the provision of medical nutrition therapy and the nutrition care process including nutrition screening, assessment, diagnosis, intervention, monitoring and evaluation, education and menu processing according to established departmental policies. Recognize and adopt activities appropriate to the specific needs of the patient/resident population and in line with the professional standards of practice. Demonstrate accountability for the proper use of patients’ protected health information. Manage and control resources and materials to ensure quality, adequacy of supply and cost control within budgetary guidelines. Maintain dietetic registration, state-required credentials and continuing education requirements. Ensure annual review of all policies and procedures to remain compliant with standards of local health authorities and regulatory agencies including the Joint Commission Standards. Monitor, order and inventory supplements. 

Other Duties and Responsibilities In the Role
Assist in planning department projects. Participate in a committee for the component (i.e., safety committee). Develop and implement an action plan to address issues. Assist in the development of an emergency response plan. Assist in human resources-related functions at the component level. Play an active role in the management of department expenses and labor. Participate in an annual peer review in a facility other than own. Develop and present monthly education to frontline staff that relates to patient menus and diet orders. Work closely with management team to develop holiday menu for patients while being conscious of standard production constraints. Manage CBORD: nutritional information and link with call center, dietitians and food allergies. Coordinate intern program across all four campuses. Serve as a clinical dietitian when work load is heavy or reduced staff due to vacancies and time off requests. 

Qualifications
The position is an experienced nutrition management role, and requires at least 3 years of management experience. Experience in managment of a complex single-site or Multiple site Hospital preferred. 

Registration Requirements
Completed a minimum of a bachelor’s degree at a US regionally accredited university or college and course work accredited or approved by the Accreditation Council for Education in Nutrition and Dietetics (ACEND) of the Academy of Nutrition and Dietetics. Completed an ACEND-accredited supervised practice program at a health-care facility, community agency, or a foodservice corporation or combined with undergraduate or graduate studies. Pass a national examination administered by the Commission on Dietetic Registration (CDR). Obtain / Maintain required state licensure to practice dietetics.

JOB DESCRIPTION RENAL DIETITIAN

The Renal Dietitian provides services to the Hemodialysis or Peritoneal Dialysis patients and their families in clinical and hospital settings to support and maximize the nutritional functioning and education of the patient.

QUALIFICATIONS: 
· Bachelors degree in Nutrition/Dietetics or similar area.
· Renal Dietitian status with the American Dietetic Association or licensed in the practice of nutrition and dietetics if required in the State where the facility is located.
· Minimum of two years experience in clinical dietetics and/or nutrition.



JOB REQUIREMENTS JOB FUNCTIONS: 

1. Assess nutritional status to determine appropriate nutrition intervention and make recommendations. Revise recommendations as needed. 
2. Review medical record and history, consult with medical staff, and the patient, document appropriate information on medical record.
3. Create an individualized diet for each patient.
4. Provide nutrition education and counseling based on individualized patient needs including nutrient requirements, knowledge and abilities, medical, economic and social situation.

Club Member Experience


· Performs individual personalized nutrition and wellness assessments
· Develops safe, professional, and comprehensive nutrition and lifestyle programs to ensure clients are satisfied with their program and remain motivated to attain their personal wellness goals
· Leads and facilitates health and wellness educational seminars and workshops
· Teaches a set curriculum of nutritional topics to our TEAM weight loss and TEAM fitness classes

· Works closely with the Personal Training staff and Club Management when designing member programs and classes
