The Department of Health and Mental Hygiene (DOHMH)’s Office of Emergency Preparedness and

Response (OEPR) seeks an innovative, driven senior staff member to direct its Exercise and Training

Unit. This forward-thinking manager should be ready to implement a system of dynamic trainings and

challenging exercises that will help DOHMH meet its key citywide roles in emergency response. The

Unit’s major activities include:

-- Coordination of key agency-wide emergency management trainings, including an initiative to train all

DOHMH employees on the Incident Command System (ICS) and their specific ICS Roles as well as indepth

leadership trainings on emergency response.

-- Development and execution of comprehensive agency-wide and multi-agency exercises, including

discussion-based exercises, functional exercises, and full scale exercises.

-- Provision of technical support to multiple DOHMH programs and healthcare entities in the

development, implementation, and evaluation of exercises and training relating to the emergency

management cycle.

-- Represent DOHMH at exercise and training meetings convened by NYC Office of Emergency

Management, including participating in the development of and participation in citywide exercises.

-- Manage budgets for consultants, equipment, and other training and exercise related expenses.



The Director of Exercises and Training will be expected to perform the following at a high level of

excellence:

• Supervise and lead several staff, including a Training Coordinator, Exercise Coordinator, and a Project

Coordinator.

• Develop, direct, and implement compelling emergency management exercises, including contract

management, setting exercise objectives, assembling key stakeholders, providing technical assistance

to ICS Emergency Response Groups, and working with DOHMH’s Evaluation Unit in developing

evaluation tools.

• Provide exercise and training technical assistance to DOHMH programs and healthcare system

stakeholders as appropriate and needed.

• Participate in external exercise planning, coordinate DOHMH involvement in external exercises,

identify and liaise with internal and external agency partners on exercise and training related matters.

• Use the ADDIE instructional design model and other best practices to identify, develop, and evaluate

training opportunities and facilitate attendance/completion.

• Coordinate with DOHMH’s Bureau of Public Health Training on Learning Management System issues

and online training.

• Facilitate grant-required exercises and training in accordance with HSEEP guidelines;

• Identify and coordinate Emergency Preparedness staff development opportunities to increase overall

awareness of emergency preparedness and response expertise (e.g.: monthly trainings,

online/classroom based FEMA courses).

• Prepare necessary grant reports and other related documents to reflect how DOHMH training and

exercise activities have made NYC more prepared to detect and respond to a public health emergency.

• Track attendance of mandatory and recommended training; and

• Work closely with OEPR’s Planning Unit and Evaluation Unit to identify training and exercise

documents based on current plans and After Action Reports corrective actions and ensure those

corrective actions have been adequately addressed.



It is expected that the Director of Exercises and Training will be a key member of the agency’s incident

command structure during emergency events, which may include 24-hour availability.



Minimum Qual Requirements

1. A baccalaureate degree from an accredited college and five years of full-time professional

satisfactory experience acquired within the last fifteen years, in a health services setting such as a

laboratory, hospital, or other patient care facility, or in a public health, environmental health, or mental

hygiene program, at least 18 months of which must have been in a managerial capacity, consisting of

managerial experience clearly demonstrating the ability to perform difficult and responsible managerial

work, requiring independent decision-making concerning program management, planning, allocation of

resources, and the scheduling and assignment of work

2. Education and/or experience equivalent to "1" above. Education may be substituted for experience on

the basis that each 30 graduate semester credits from an accredited college in hospital administration,

public health, public administration, business administration, management or administration can be

substituted for one year of non-managerial experience up to a maximum of 60 semester credits for two

years. However, all candidates must have a minimum of a baccalaureate degree and 18 months of

managerial experience as described in "1" above.



Preferred Skills

The candidate should have proven experience conducting emergency preparedness training and

exercises and operating within an Incident Command System environment. L449 or E449 ICS Trainer

Certificate preferred. Working knowledge with the Department of Homeland Security’s Homeland

Security Exercise and Evaluation Program (HSEEP) strongly preferred. The candidate should have a

Certificate in Training (e.g. FEMA Master Trainer, Training Manager, Municipal Police Instructor) from a

recognized industry institution. The candidate must have exceptional communications and interpersonal

skills, and be able to manage a large number of projects simultaneously under pressure with very tight

deadlines. The candidate should have managerial experience. Candidate must have very strong writing

skills and presentation skills. Experience working with New York City Agencies is preferred. A Master

Degree in Public Health/Public Administration/Business Administration, Education, Emergency

Management, or equivalent (including military equivalents) is strongly preferred.

JOB DESCRIPTION

The Registered Dietician (RD) reports to the Nutrition Team Manager to conduct nutritional assessments and determine dietary needs for a frail adult population to achieve an optimal level of independent functioning. The RD provides necessary consultation and teaching to members, families/caregivers, and the care teams of Senior Health Partners (SHP) teams and Healthfirst Complete Care, and other integrated products, to ensure the mission of the program is maintained.

The RD frequently communicates complex information and interacts through electronic documentation/tools. This is a paperless work environment requiring daily hands-on administration of multiple electronic Patient Health Information (PHI) databases and security requirement tools such as encryption. These systems include, but are not limited to CCMS (Care Enhance Clinical Management Software), Sun guard Maces Service Module (electronic archiving), Right Fax, VoIP, Virtual Work Platforms (VPN), scanning and creating PDF files (Adobe Acrobat), and MS Office 2010 software (such as Word and Outlook).

JOB REQUIREMENTS

• Deliver and direct nutritional care and services for members in collaboration with Care Management Team (CMT) offering nutritional counseling and education through case findings and nutrition screening.

• Evaluate members’ nutritional status utilizing various assessment tools and obtains primary care physician’s orders as required. Initial evaluation, reassessments and/or referrals from CMTs will be completed by phone or through a home visit.

• Calculate nutrient intake integrating the nutrition treatment plan into the overall care plan; collect laboratory data, and consult with the Care Management Team, Nutrition Team Manager, Clinical Manager and Medical Director as appropriate; assess food preferences, preferred methods of preparation including cultural and environmental factors influencing dietary intake.

• Educate members and care givers about the need for and preparation of therapeutic diets as part of the care plan.

• Collaborate with Care Management Teams through daily communication and team meetings to develop an optimal plan of care for member(s).

• Monitor and sign-off assessments from Diet Technician(s)/Diet Technician Registered(s) ensuring an appropriate care plan, and pertinent documentations are completed in an accurate and timely manner.  Assist with providing preceptor and training accordingly.

• Work with Care Management Teams to support the company's mission and program goals by providing pertinent information and coordinate between dietary/other services, participating in  care planning meetings including formulating and implementing policies/procedures.  Participate in quality management activities and educational programs designed for members, families/caregivers and staff.



Minimum Qualifications:

• Bachelor's degree and Registered Dietitian in good standing.

• Fluent in speaking Spanish.

• Related industry experience.

• Advanced clinical and home health care knowledge.

• Technical experience managing patient health information (PHI) in a database module and archival system (e.g. Macess).

• Thorough knowledge of current community health practices for adults.

• Working knowledge of the interdisciplinary model of care management.

• Proficiency in navigating internet browsers, computer hardware and multi-tasking with multiple electronic documentation systems (toggling).

• Strong proficiency in Microsoft Word (documenting, writing procedures, recommendations, etc); Excel (sorting, tracking, formulas, pivot tables, etc); time tracking systems; familiar with Corporate email/calendar system; managing electronic patient health information (PHI).

• Meet Requirements for State registration with the Uniform Assessment System (UAS): Unexpired NYS driver’s license or NYS non-driver’s license ID (DMV Identification). If residing outside of NYS, candidate must obtain a NYS non-driver’s ID before commencement of employment. State registration with UAS for long-term care in NY State is required to access for training, testing and system usage. Upon completion of UAS training, the State will administer testing online. If a passing score is not achieved, the State will allow the individual to retake the test once. The UAS access is a necessity for this position, so employment is contingent on acceptance and continued authorization by the State to have UAS access.

Telecommuter requirements:

Ability to work remotely is dependent upon completion of training and passing audit scores, which varies by department. It is also contingent on the following telecommuter requirements:Â

• Designated work area at the Telecommuting location away from the main flow of home traffic which meets OSHA and departmental policy.

• Broadband solution will be a cable modem type using an Ethernet connection from a reputable provider such as Comcast, Cox Cable or Cablevision, TW or fiber service such as FIOS or AT&T Uverse offered by Verizon, AT&T or regional partners.Â

• Usage of a DSL broadband is not recommended and will not be supported by Healthfirst’s Technical Services team, additionally; wireless is also not recommended with both options requiring prior approval of the division Vice President and Human Resources.Â

The Nutrition Team Manager at Senior Health Partners (SHP), a Managed Long-Term Care Plan (MLTCP), is responsible for supervising a dietary team consists of Dietary Technicians, Dietary Technician Registered, Registered Dietitians in conducting nutritional assessments to determine dietary needs for a frail adult population to achieve an optimal level of independent functioning. Provides necessary consultation and teaching to members, families/caregivers, and the Senior Health Partners (SHP) teams/HF Completecare teams that ensure the mission of the program is maintained.

This requires auditing and recommending corrections of electronic individualized recommendations for all Members. Responsibilities include, but are not limited to, supervision of daily clinical and administrative operations while auditing staff performance and Member database(s) detailing Personal Health Information (PHI) entered by staff. Also certifies compliance with applicable NYS regulations.

JOB REQUIREMENTS

• Supervises, directs, and evaluates approximately 5-10 staff members in the dietary team.  Conducts prospective new hire interviews, conflict resolution, and performance evaluations for each staff member.  Provides instruction(s), continued system(s) training, and supervision to direct reports while ensuring all staffs are delivering and directing nutritional care and services for members in collaboration with Care Team (CT) offering nutritional counseling and education through case findings and nutrition screening.  Maintains appropriate staffing levels guaranteeing safe and effective care management. Recommends staffing levels for planning and budget development. Routinely evaluates staff member's clinical skills and technical comprehension. Works with Assistant Directors, Senior Management and Clinical Staff to provide orientation and in-service programs for staff to meet regulatory requirements, support performance improvement, and are in alignment with SHP and CC’s workflow and technical systems. Guarantees integration of the nutrition treatment plan into the overall care plan; collect laboratory data, and consult with the Care Team, Clinical Manager and Medical Director as appropriate. Supervises and audits compliance, including HIPAA standards, for confidentiality and management of PHI plus CMT entries in varied electronic PHI storage systems (e.g.: CCMS, Macess, and EMR). Frequently communicates complex information and interacts with management through electronic documentation/tools. Fosters staff in delivering superior customer service and negotiating skills.

• Supervises the continuation, coordination and delivery of care management, individualized care plan(s) and authorized services to Members by CMTs through database progress reports.

• Collaborates daily with CMT to safeguard the appropriateness and effectiveness of services recommended/utilized such as informing of equipment maintenance needs, coordinating information between dietary/other services, participating in care planning meetings including formulating and implementing policies/procedures.

• Participates in Quality Management Program by identifying improvement opportunities, including performance, through meticulous documentation of clinical services. Assists in supplying metric data for quality studies by performing daily routine PHI audits in database and archival systems, and recommends studies for the annual quality plan. Supports Quality Management Program by accessing and interpreting management reports and using said data to ensure accuracy.

• Provides on-call coverage at a minimum of one full-week (24/7) at least 1x per year to assist in troubleshooting, teaching, and coordinating the delivery of care outside of core business hours (8:30AM- 5:30PM, M-F) for all SHP CMTs. The delivery of care may also require participation on-site and off-site via inter-agency and family meetings as deemed necessary.Â



Minimum Qualifications:

• Bachelor's degree and Registered Dietitian in good standing. Unexpired NYS driver’s license or NYS non-driver’s license ID (DMV Identification). If residing outside of NYS, candidate must obtain a NYS non-driver’s ID before commencement of employment.

• State registration with the Uniform Assessment System (UAS) for long-term care in NY State is required to access for training, testing and system usage. Upon completion of UAS training, the State will administer testing online. If a passing score is not achieved, the State will allow the individual to retake the test once. The UAS access is a necessity for this position, so employment is contingent on acceptance and continued authorization by the State to have UAS access.

• Related industry experience.

• Advanced clinical and home health care knowledge.

• Working knowledge of the interdisciplinary model of care management.

• Thorough knowledge of current community health practices for the frail adult and working with cognitive impairments particularly in Senior population.

• Technical experience managing patient health information (PHI) in a database module and archival system (e.g. Macess).

• Experience leading others through assignments and projects including delegation of responsibilities and achieving deadlines.

• Excellent written communication and the ability to document grammatically correct notes.

• Excellent communication skills and demonstrated ability to work well with members and their families in dealing with sensitive issues. Communicate results in a succinct and clear written manner.

• Ability to facilitate problem-solving, manage many details and handle emergency situations is essential.

• Proficiency in navigating internet browsers, computer hardware and multi-tasking with multiple electronic documentation systems (toggling).

• Intermediate Microsoft Word, Excel, time tracking, Corporate email system and managing electronic patient health information (PHI).

• Telecommuter requirements:Â Direct Ethernet FIOS or Cable internet connection (wireless connection not permitted).

• Random work activity audit reports will be conducted for all telecommuting employees analyzing performance and system activity on a given day.

• While performing the duties of this job, this employee will be occassionally lifting/carrying and pushing/pulling 1-20lbs. Will occassionally attend seminars, training etc. and may be required to transport print materials.
