Clinical Dietitian position - Full Time, Brooklyn, NY, Long Term Care 


Minimum Qualifications: 

• Baccalaureate Degree with major in dietetics or food and nutrition from a program approved by the Academy of Nutrition and Dietetics (formerly American Dietetic Association) 

• Registration or eligibility by the Commission on Dietetic Registration (CDR) as a Registered Dietitian. 

• Minimum of three years of clinical experience. 

• Attends continuing education programs to keep abreast of new guidelines in the field. 

• All position are subject to a background investigation. 

Responsibilities: 

• Performs nutrition assessments and reassessments daily as per assigned unit, admission date and time as per the current policy of the department. 

• Visits residents to obtain a detailed nutrition history summary and determine their nutritional needs. Once needs are determine their nutritional needs. Once needs are determined, comprehensive assessments will be completed including recommendations that will be communicated to physicians and/or nursing staff. 

• Performs and documents meal rounds to observe residents acceptance of meals. Once observed, actions to correct the residents' concerns regarding their meals should be taken which includes obtaining likes and dislikes and allergies and communicating them with the kitchen and entering them into the Geri Menu System. 

• Provides nutrition education and counseling to residents (and family if appropriate) identified at nutritional risk prior to discharge. Education is to be reflective of the resident's nutritional needs and in accordance with therapeutic diet guidelines. 

• Participates in discharge planning rounds, interdisciplinary team rounds, Wound Care Rounds on assigned unit to discuss the plan of care and recommendation with the interdisciplinary team. 

• Reviews daily census sheets and monthly MDS report to determine which residents need to be seen. 

• Documents all assessments, reassessments, likes, dislikes, allergies and all other pertinent information in medical record. 

• Documents daily productivity of all patients assessed and/or visited and any patient associated meetings daily. Provide daily productivity reports to the clinical nutrition manager on a weekly basis. 

• Attends and participates in department monthly meetings - may plan and conduct meetings in the Clinical Nutrition Manager's absence. 

• Performs other related duties as assigned.

Consultant Position: Program/Site Review Consultant with United Way of New York City - Hunger Prevention and Nutrition Assistance Program (HPNAP)

General Responsibilities:
The Program/Site Review Consultant will conduct on-site program reviews of emergency food programs (EFPs) funded by United Way of New York City - Hunger Prevention and Nutrition Assistance Program. Program reviews include nutrition, food safety, and administrative components. Consultants will be trained on how to evaluate each component using standardized site monitoring forms and data entry into the FeedNYC.org database. Program reviews are conducted during EFP hours of operation and will be assigned based on consultant’s stated availability. This consultant position requires at least 15-20 hours per month with variable hours from October 2014 through May 2014. Consultants will receive $19.50 per hour.

Specific Duties:
• Conduct on-site program reviews of UWNYC-funded emergency food programs during hours of operation.
• Schedule program reviews directly with EFP contact person by phone or email 1-3 weeks prior to visit.
• Use site monitoring forms to assess EFPs for adherence to nutrition, food safety, and administrative requirements.
• Provide written and verbal technical assistance regarding nutrition, food safety, and program administration during site visit.
• Assess needs for food service equipment based on food safety and sanitation issues and make recommendations as required.
• Complete site monitoring forms and data entry into FeedNYC.org within two weeks of conducting a program review.
• Attend a paid training in October and additional paid trainings, as required.

Job Requirements:
• Knowledgeable about community nutrition, food safety, food service, and hunger as they relate to New York City residents.
• Have professional or academic experience in any of the above topics.
• Excellent interpersonal and communications skills.
• Ability to work independently with minimal supervision.
• Ability to travel city-wide on public transportation or in own vehicle.
• Ability to work a minimum of 15 hours per month from October 2014 to May 2015 or until assigned program reviews are completed.
• Ability to communicate in a timely manner with UWNYC about status of program reviews and other pertinent information.
• Ability to work Saturdays and Sundays. 
• Strong preference will be given to candidates currently holding a NYC Food Protection Certificate. Candidates without this certification will be required to take the course within the first month of hire on their own time and without remuneration.

DATA MANAGER for a CMS Health Care Innovation Award at Mount Sinai
The Department of Geriatrics and Palliative Medicine at Mount Sinai is seeking a full-time Data Manager. The qualifying individual should have at least a master’s degree in a relevant discipline such as statistics, biostatistics, public health or epidemiology, and a minimum of three years’ experience. Experience working in the public health field and in program evaluation is preferred. Strong skills and demonstrated proficiency in SAS programming and significant experience in database management and development is a must. The position involves data management activities such as documentation and reporting, as well as quality improvement for a program evaluation data system at Mount Sinai School of Medicine.

Tasks will include programming in SAS; providing assistance in the preparation of documentation, such as detailed code books, protocol documentation, and instruction sheets. These tasks will result in the creation of aggregate datasets and associated documentation. In addition, the candidate shall create tables and graphs for reports and presentations. While SAS software is used primarily for data management, data may be received in a variety of other formats with which the data manager should be familiar. The Data Manager will be assigned other duties as necessary. Excellent verbal and written communication is a must. The chosen candidate will work in a team environment and must be self-motivated.
Required: Master’s degree ideally in mathematics, biostatistics or public health. Expertise in SAS programming, including macro and array processing and at least 3 years of experience.

Community Health Educator - HIV/Aids

A facility that provides direct services for people living with HIV/AIDS, and for those at risk of becoming infected, is seeking a Community Nutrition Educator.
POSITION SUMMARY
The Community Nutrition Educator (CNE) is responsible for the coordination of group and individual nutrition health education, the review of client assessments, referrals and program eligibility, the tracking of client progress in the program. 1. Adheres to funder and agency requirements for the provision of Nutrition Health Education (NHE).

3. Reviews Registered Dietician (RD) assessment and dietary recommendation to determine food service client receives and type of health education appropriate for client needs. 
4. Responsible for maintaining client records and works as part of Continuous Quality Improvement (CQI) and evaluation team for program improvement; Maintains client records that document attendance, participation, progress, accomplishments, and additional needs, referrals and goal.
5. Monitors, in conjunction with the Program Director, the appropriateness of the food service provided to the client.
6. Establishes connections and communicates frequently with Medical Nutrition Programs/Practitioners and RDs and makes referrals as necessary and appropriate.
7. Completes NHE and Food program screening form which includes the reasons for client participation in the program
8. Ensure clients' adherence to all program policies and procedures and follows program protocols for participation, pantry bag distribution and incentives.
9. Acts as Team Leader to conduct community coordination activities.
10. Communicates and collaborates with Funder and responds timely and appropriately to all requests from funder, Program Director and Executive Director.
11. Attend and successfully complete all required training and represent the agency at various venues promoting the program and the agency.
12. Perform other duties as assigned by the Program Director.
OTHER POSITION FUNCTIONS
1. Serves as a liaison between Iris House and program providers and partners.
2. Demonstrates teamwork and open communication and has good interpersonal skills.

Services & Advocacy for GLBT Elders (SAGE) is the country's largest and oldest organization dedicated to improving the lives of lesbian, gay, bisexual and transgender (LGBT) older adults. Founded in 1978 and headquartered in New York City, SAGE is a national organization that offers supportive services and consumer resources for LGBT older adults and their caregivers, advocates for public policy changes that address the needs of LGBT older people, and provides cultural competency training for aging providers and LGBT organizations, largely through its National Resource Center on LGBT Aging and its growing social entrepreneurship project. With offices in New York City, Washington, DC and Chicago, SAGE coordinates a growing network of 27 SAGE affiliates in 20 states and the District of Columbia.
SAGE was recently awarded funding by the New York City Council to expand its Innovative Senior Center model for LGBT older adults to four additional sites: Bronx, Brooklyn, Staten Island and Harlem.The program's theory of change is based on a whole-person wellness model and seeks to provide specific interventions targeted to the improving outcomes in each of the domains of wellness based on the needs of the specific communities and/or individuals involved. Under this Project, each new site must implement the model and meet specific contract deliverables as defined by the Department for the Aging. SAGE is now recruiting two (2) Program Managers for this Project. These positions report to the Director of SAGE Centers and are full-time.
Essential Functions:
These managers will be responsible for developing and delivering new programming with measureable outcomes based on the unique needs of the community. S/he will need to engage in site-specific needs assessments, program design and outcome measurement.S/he will collaborate with Project staff; contracted instructors and volunteer teachers to ensure innovative interventions with measurable outcomes are designed and delivered to LGBT elders.S/he will deliver programs and coordinate the work of other teachers/trainers contributing to the overall wellness of LGBT older adults.
• Health & Wellness Manager: The Health & Wellness Manager will enhance the wellness of LGBT older adults by creating and implementing health, nutrition and exercise programs at SAGE ISCs across New York City that respond to their needs and lead to healthy, active aging in their community.
Key Responsibilities:
• Understand and guide the effective implementation of the ISC program model across sites;
• Program design and implementation, including conducting needs assessments, designing specific or tailored interventions to meet those needs, and program evaluation and monitoring;
• Develop interventions that can potentially lead to new evidence-based practices;
• Deliver and/or train site staff or consultants to deliver new interventions under the program model;
• Identify and recruit consultants/teachers/service providers with expertise to deliver innovative, culturally competent, evidence-based programming at various sites;
• Ensure sufficient quantity and quality of programming at each site to meet program outcome and contract deliverables;
• Collaborate with Project staff to facilitate cross-site learning and sharing of best practices;
• Support site managers in the development of processed and systems to ensure timely collection of client and service delivery data and related data entry;
• Complete administrative paperwork and assist in DFTA reporting process;
• Represent SAGE ISCs at community events (e.g. health fairs, pride festivals); and
• Other tasks and duties as assigned by the SAGE Center Director.
Required Skills/Qualifications:
• A master's degree in social work, public administration or equivalent degree in a related field, plus a minimum of 5 years' experience in a non-profit setting. Substantial work experience (7+ years) in non-profit program development and management, with a focus on aging and/or LGBT communities may be substituted for master's degree;
• Entrepreneurial, independent thinker who is self-directed, creative and able to complete tasks with limited supervision;
• Experience with effectiveness-based program development and implementation;
• Capacity and a desire to gain new skills and knowledge to drive innovative programming;
• Understanding of performance measurement and management;
• Ability to manage multiple projects, prioritize activities and meet deliverables under deadlines;

The purpose of this program is to teach basic culinary skills to individuals with developmental disabilities and increase their potential for living independently one day.

Hours: 5 (2 in class; 3 hours divided by course prep and weekly grocery shopping for class)
Description: We are looking for a part-time employee to work 5 hours a week dedicating their time to running the Chefs R Us Cooking Program. The Culinary Instructor will facilitate a 2 hour cooking course once weekly with adult individuals with developmental disabilities. The instructor will create a lesson plan, addressing specific skills to be acquired. The instructor will be allotted a program budget and will be expected to do all shopping for weekly cooking course. Three paid hours are allotted for lesson planning and shopping. The individual can use the time as they see fit for making the program successful each week.
The ideal candidate will have a background in culinary arts and education. Having experience working with individuals with developmental disabilities is not required, but would be a major plus. Having experience in nutritional science is also not required, but would be a major plus. The ideal candidate will also be savvy with saving money and sticking to a budget.
We are looking for a motivated and enthusiastic individual who can inspire the individuals in our program to enjoy aspects of cooking and healthy eating. The instructor must pay particular attention to dietary restrictions and be aware of client allergies that would affect meal plans. The instructor must also be prepared for each class at least 48 hours in advance in order to notify the program what meal plan will be prepared in class. This is necessary to give our staff time to notify parents and families and receive permission for them to participate in the class.
The instructor will report to the Special Project and Clinical Coordinator. The instructor will also collaborate with the Director of the Day Program.
