Position Summary:

• Responsible for planning, coordinating and providing diabetes-related health promotion activities in the YK Delta Region.

• Provides comprehensive diabetes education and care for patients with Type 1 DM, Type 2 DM, Gestational DM, IFG, Pre-Diabetes, etc in a clinic setting.

• Patients and activities are accessed through multiple village trips per month.

• Work closely with village clinic and hospital staff to promote diabetes standards of care and inform of new advances in diabetes care.

• Assists Diabetes Coordinator with maintenance of the Diabetes Registry.

• Works closely with the Diabetes Case Manager to ensure comprehensive care of the patients.

General Purpose:

To plan, organize, develop and direct the overall operation of the Dietary Department in accordance with the current federal, state and local standards governing the facility, and as may be directed by the Administrator, to ensure that quality nutritional services are provided on a daily basis and that the Dietary Department is maintained in a clean, safe and sanitary manner.

Essential Job Functions (With or Without Reasonable Accommodation):



Dietary Duties:

To supervise and direct the Dietary Department and to plan, develop, organize, implement, evaluate and maintain its programs and activities; ensure an ongoing quality assurance program; coordinate Dietary services and activities with Nursing Services and other related departments; work to develop, review and plan normal, special and therapeutic diet plans for residents; ensure meals are prepared and delivered in on a timely basis; ensure meals are prepared in accordance with menu and diet preferences and established portion control procedures; maintain proper records to ensure that residents' diets are in compliance with physicians' orders; ensure proper storage of raw and leftover foods; inspect and maintain storage rooms, utility rooms and other areas of the Dietary Department and ensure control of equipment and supplies.



Personnel Duties:

Responsible to recruit, interview, hire, counsel, discipline and terminate dietary personnel; document and coordinate personnel actions; schedule department work hours including vacation and holiday schedules, work assignments, etc; to expedite work, delegate administrative authority, responsibility and accountability to other dietary personnel as deemed necessary to perform assigned duties and coordinate services to ensure that daily Dietary services can be performed without interruption.

Inventory and Planning Duties:

Recommend to the Administrator the food, equipment and supply needs of the Dietary Department; responsible for the ordering and purchase of food, equipment and supplies as necessary, including interviewing and dealing with food, supply and equipment vendors. Develop and monitor adequate and cost effective inventory control procedures; make regular inspections of food supplies and equipment to ensure that adequate levels are maintained and that equipment is available and working properly. Forecast needs of the department; prepare and plan the Dietary Department's budget for food, equipment, supplies, and labor and submit to the Administrator for review; and maintain current written records of department expenditures.





Residents' Rights Duties:

Maintain resident confidentiality; treat residents with kindness, dignity and respect; know and promote Residents' Rights; monitor dietary services to ensure that resident's dietary needs are met; ensure that all dietary service personnel are knowledgeable concerning residents' nutritional needs; maintain written records of all resident food preferences, complaints, and grievances.

Care Plan and Assessments Duties:

Under the guidance of the Consultant Dietitian , developing for each resident preliminary and comprehensive assessments of the dietary needs and written dietary care plans that identify the dietary problems and/or needs of the residents and the goals to be accomplished for each dietary problem and/or need identified; ensure that all dietary personnel are aware of the care plans and that care plans are used in providing daily dietary services to the resident; review nurses' notes and monitor residents' weights to determine if the care plans are being followed and if the resident' needs are being met and identify and monitor high-risk residents ensuring that any special needs and requirements are met.





Safety and Sanitation Duties:

Maintain safety standards and follow written aseptic and isolation techniques relating to residents, personnel and hazardous conditions or equipment; ensure cleanliness of Dietary work areas, storage rooms and preparation areas; ensure that Dietary personnel understand and follow established infection control procedures when isolation precautions become necessary, Universal Precautions, established safety rules, fire and disaster procedures, departmental policies and procedures and maintain habits of good hygiene; ensure that in your absence all dietary personnel report any hazardous conditions or equipment to the Assistant Food Service Director and/or the Administrator.



Staff Development Duties:

Attend, direct and participate in orientation, training and inservice educational activities.



Administrative Duties:

Attend and participate in various committee meetings; solicit and consider advice regarding the Dietary Department from the residents and other departments; meet with Dietary personnel on a regularly schedules basis and insure that quality control measures are continually maintained.

1. Completion of undergraduate education to include an associate degree (AAS) in Food Service Management and/or be a qualified Dietary Manager required

The internship program provides an opportunity to gain first-hand experience in dietetic counseling, media/marketing and business/entrepreneurial skills.
