Sodexo is seeking a Registered Dietitian with at least 2 years experience (at least 1 year lead or management experience) to manage the Clinical Nutrition program at Goldwater Hospital. This hospital has about 200 LTAC (Long Term Acute Care) patients with an average LOS of about 6 months. About 1/2 of the LTAC patients are on vents requiring nutrition support. There are also about 165 long term care residents at Goldwater Hospital.

In this position, the candidate will

• Oversee a staff of 5 Clinical Dietitians and manage the diet office.

• Represents the department on hospital-wide medical staff committees.

• Demonstrate skills in managing and developing clinical staff; development of policy and procedures; accreditation and regulatory readiness and review; and establish and maintain standards of care.

• Have union experience

• Have excellent communication and customer service skills, have a track record of proactive interaction with the medical team, be an outstanding team player, and have strong demonstrated leadership skills.

• Be a Registered Dietitian with clinical experience in both long term care and acute care

• Be familiar with electronic charting

1. Creation of disease-specific dietary management plans for infants, children, adolescents, and adults with inborn errors of metabolism in the outpatient and inpatient settings.

2. Performs ongoing nutritional assessments on patients to determine nutritional status, individualizes, monitors and updates nutritional care plans based on laboratory data and clinical findings to optimize nutritional intake and maintain appropriate growth. Consults with physician whenever necessary.

3. Interview patients/family members to obtain information regarding the patient's nutritional status, food habits and preferences to assist with individualizing patients' nutrition care plan.

4. Discusses dietetic goals and barriers with patients and their families and counsels patients and /or their families individually or in groups, on ways to meet dietary adherence in a long range.

5. Works closely with other metabolic dieticians, attending physicians, nurse practitioner, clinical coordinator, residents and social worker. Includes participating in patient care conferences and results meeting.

6. Participates in the development and revision of nutritional educational materials to improve patient quality of care specific for diagnosis, age and literacy level.

7. Maintains professional growth and development through continuing education, professional meetings and conferences to keep informed of current practices and trends in fields of dietetics and nutrition. Applies current practices to nutritional care.

8. Demonstrates organization in performing several tasks simultaneously. Shows willingness to move from one task to another as work requires without being asked.

9. Extends nutritional care to patients reporting laboratory results via phone and/or email with treatment plan and recommendations.

10. Ensure customer satisfaction inclusive of replying to all emails and/or phone calls placed by patients and/or family members regarding nutritional care on a daily basis.

11. Prepares all prescriptions needed for metabolic formulas and Kuvan.

12. Documents in the medical record all ongoing nutrition interventions.

13. Receives interns from the internship/preceptorship of hospital program

14. Projects a positive image in the community.

In 2005, as a direct response to the public health crisis of obesity in our youth, HCZ created an effort called the Healthy Living Initiative (HLI) which encouraged our students and their families to incorporate healthy food choices and physical activity into their daily lives. In the years since then, we have sought to deepen our work in this area, and have developed a strategic program model and growth plan and renamed this initiative Healthy Harlem, which consists of best practices in physical activity, nutrition education, and community support.

Like HLI, Healthy Harlem is designed to instill a culture of healthy living throughout the Zone, but is more expansive and strategically focused in the way we reach youth in our programs, their parents/care givers, and the community at-large. Our three-pronged approach consists of Prevention, which serves the general youth population and their families; Treatment, which provides intensive support for overweight and obese youth and their parents/caregivers; and Medical Monitoring, which connects severely obese youth and their families to professional health care providers.

We are seeking part-time Health Educators to work at selected HCZ sites. They will work at a designated program location and will be part of a wellness team dedicated to improving nutrition and increasing physical activity for students at their respective site.

Essential Duties and Responsibilities

• Conducts behavioral treatment sessions/groups for youth and parents;

• Conduct health assessments to determine overall health status of overweight and obese youth

• Provide a range of supportive and educational health services

Breakfast Coordinator, NYC No Kid Hungry Campaign (Temporary)



No child should grow up hungry in America. Since 1984, Share Our Strength® has led the fight against hunger and poverty by inspiring and organizing individuals and businesses to share their strengths. Share Our Strength, a national nonprofit, is ending childhood hunger in America by ensuring all children get the healthy food they need by connecting kids in need with nutritious food and teaching their families how to cook healthy, affordable meals, the No Kid Hungry Campaign surrounds children in this nation with healthy food where they live, learn and play.



Responsibilities:

The Breakfast Coordinator for the NYC No Kid Hungry Campaign will be responsible for executing an outreach plan created by a coalition of NYC non-profit, advocate and agency partners at the School Meals Coalition Outreach Working Group to build awareness about the school breakfast program and ultimately increase participation in school breakfast in approximately 10-15 targeted K-12 public schools in Washington Heights, Manhattan. Those 10-15 schools represent the schools the No Kid Hungry Campaign will target within Washington Heights while the other partners at the Coalition are executing the plan to their targeted subset of the roughly 60 schools in the same district. The Coordinator will identify barriers that exist to increased breakfast participation and work to overcome those barriers with each designated school. The Breakfast Coordinator will establish contacts, build relationships, and convene meetings with relevant stakeholders and will help coordinate efforts related to this project between the other partnering organizations of the School Meals Coalition Outreach Working Group, and the NYC No Kid Hungry Campaign.



• Responsible for increasing breakfast participation in each of the targeted schools and creating lasting relationships within the school and community.

• Organizing outreach to all relevant stakeholders including school principals, food and nutrition directors, superintendents, PTA leaders, teachers, elected officials, parents, and other community members within the targeted district.

• Tracking meetings and all follow-up contact, actions and communications, and maintaining these within a database.

• Coordinating the distribution of outreach materials, including toolkits, videos, student surveys, brochures, mailers, and presentations.

• Providing technical assistance to schools to address issues related to school breakfast, including one-time costs, impact on learning environment, etc.

• Collecting and compiling data from participating schools to track progress and provide impact stories on the program's success.

• Evaluating project grant applications and monitoring grant use to ensure effective implementation.

• Maintaining relationships and contacts with staff within state agencies and other anti-hunger policy groups to coordinate efforts as needed.

• Developing and coordinating outreach with other audiences, including members of the faith-based community and elected officials, to educate them about opportunities to contribute to ending childhood hunger through the No Kid Hungry Campaign.

• Identifying and cultivating supporters or champions for School Breakfast and encouraging their involvement in the future.

• Other duties as assigned related to supporting the project.

Ability to write letters, summaries and reports, using prescribed format and conform to all rules of punctuation, grammar, diction and style. Ability to work with mathematical concepts such as probability and statistics. Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations. SUMMARY: Working with the Executive Director of the Sackler Institute and other members of the Academy to support the activities of the Institute including working with experts, working groups and advisory councils, organizing scientific conferences, managing partnerships and publications.



PRIMARY ACCOUNTABILITIES:



Oversee the day-to-day activities of the Sackler Institute for Nutrition Science, including working with experts, advisory councils, and working groups; network with stakeholders in the field, NGOs, and private industry; develop spreadsheets of milestones, and monitor and report on progress.

Manage and adhere to annual program budget. Author grants for program support from government agencies, private companies, medical education, foundations, and individuals. Prepare and negotiate budgets and Letters of Agreement with partners and grantors.

Develop and maintain budgets and discuss financial obligations with sponsors and partners. Write/manage grants and contracts, progress, follow-up reports, and budget reconciliations.

Manage multi-stakeholder partnerships, develop and implement a system for regular updates, active engagement and reporting

Create strategic alliances and nurture close ties with a network of leading organizations, researchers, and institutions in academia, government, and industry; collaborating with them to develop outcome-oriented and impactful programming focused on key scientific questions and critical issues.

With the Conferences team, manage the development of nutrition conferences from idea through to completion, including post-conference reports and educational dissemination materials.

Review applications for research grants and awards, coordinate the work of external review committees, and select individuals for Research Grants, Poster Presentations, Travel Fellowships, and Poster Prizes. Write and review reports for supporters.

Monitor and manage information, discussion and developments in the areas of nutrition science, scientific policy around nutrition, and food safety. Compile and analyze scientific data for presentations, reports and articles.

Work closely with and support the activities of the Academy staff including, but not limited to scientific programs and Annals.



REQUIREMENTS: Ph.D. in nutrition, global health, public health or related field and 3-5 years post-doctoral experience in an academic, research or a programmatic setting. Program, staff and administrative management experience is required. Excellent computer skills and familiarity with distance communication and learning tools (e.g. WebEx) required. Experience with research methodology and statistical packages, and evidence of scholarly activities such as peer-reviewed publications and scientific presentations is required. United States Federal or other successful grant writing and award management experience preferred. Must be able to travel 10-15% internationally as needed.



The successful candidate will be highly organized, with excellent communication/networking skills and have a demonstrated ability for a broad scientific perspective with the ability to work independently and as part of a team. Qualified candidates will bring demonstrable project management experience as well as evidence of success working in a complex, integrated organization across many programmatic and functional departments.
