Premise Health runs the Wellness Center at NYLife and as the Nutritionist I am looking for a student to help me manage weigh-ins for our 2016 Weight Loss Challenge.



We will be doing weekly weigh-ins on Tues and Friday mornings from 9-11am for 10 weeks starting Tues 1/12 (through 3/18).



I would love to have an intern who is available every Tues and Friday from 9-11am for the entire 10 weeks.  I can be flexible regarding your time commitment.



 I am willing to help with any school credit forms and if additional time is needed for school credit, I can provide!



I will also be doing 3 lunch and learns during the weight loss challenge which will be at noon, I have not selected the dates yet, but would be open to having an intern help with developing and if they are interested, in presenting.  These lunch and learns are “in-person” and also done as webinars.



I prefer if you have experience working with Excel.



This is a great experience for someone who is looking to get some insight into worksite wellness.

Responsibilities: The Bureau of Child & Adult Care Food Program (CACFP) is a federally funded program which provides nutrition education and reimbursement for healthy meals and snacks served to children in family day care homes, day care centers, emergency shelters, afterschool programs and adults in day care. The Public Health Nutritionist 1 (PHN1) will provide nutrition expertise and technical assistance to ensure the quality and safety of food service operations at participating organizations. On-site visits conducted by the PHN1 will also monitor and evaluate fiscal and administrative integrity and ensure compliance with state and federal regulations. Responsibilities of the PHN1 may include: reviewing and recommending approval of applications and providing training related to menu planning, food service operations, obesity prevention programming and maintaining required records consistent with CACFP regulations. The PHN1 may also supervise support personnel.



Contingencies of Employment: Contingent-permanent, full-time. Occasional statewide travel, including overnights and travel to areas that may not be served by public transportation may be required. 70.4 transfer and provisional candidates will be required to take and pass the next holding of the Civil Service PHN 1 examination. Responsibilities include: Manage HPNAP contracts located in the Capital Region of NY State. Ensure contractors’ compliance with grant deliverables and program policies; negotiate contractor budgets and work plans. Responsibilities:

•  Manage nutrition staff and provide individual and group supervision to a team of dietitians

•  Collaborate with Program Director to determine client treatment trajectories

•  Coordinate care with multidisciplinary outside treatment providers

•  Facilitate nutrition groups and meals for adults and adolescents

•  Oversee and develop nutrition groups and menus for all programs

•  Conduct intake nutrition assessments for potential clients for program

Duties Description As a Dietitian 2, you would be a member of an interdisciplinary team and provide Medical Nutrition Therapy which may include consulting with physicians on nutrition support requests; planning and supervising the preparation of special diets and menus; maintaining safe and sanitary conditions; and training or assisting individuals, families and staff to plan specialized and therapeutic nutrition care plans. You may have oversight responsibility in the service of food; assist in the instruction of students or nutrition interns in Medical Nutrition Therapy and food preparation techniques, supervise dietetic interns and technicians; and/or supervise a small food preparation service unit.

We are looking for a Full Time dietitian for our Brooklyn location.



SCOPE OF PRACTICE

To obtain vital nutritional, socioeconomic and health information for the purpose of determining the nutritional status of the dialysis patient.



RESPONSIBILITIES

Accountable for the overall nutritional health and well-being of patients receiving maintenance dialysis treatment at the Dialysis Center.

Monitors patients’ progress to determine dietary compliance through review of lab data, chart notes, and oral and written communication with the attending physicians.

Instructs patients and families on prescribed renal diet and makes alterations in dietary patterns according to the changes in patient’s condition.

Participates in multi-disciplinary health care conference in which the total health care of the patient is discussed.

QUALIFICATIONS

Possession of a B.S. Degree in Foods and Nutrition from an accredited college or university and meets the eligibility requirements for membership in the American Dietetic Association.

Passage of registration exams given by the Commission on Dietetic Registration resulting in the attainment of Registered Dietitian (R.D.) status.

One year experience in clinical nutrition after registration as a dietitian.

The program will also include how to write a plan of correction for citations and a review of the proposed regulatory changes that will impact long-term care specifically those that relate to food and nutrition.

EXOS Works is currently looking to add a Senior Performance Dietitian to join our Corporate Wellness team. Only candidates who are currently RD certified will be considered. This person must be results driven, able to perform the following job requirements and possess the following skills and credentials:



• Perform nutrition evaluations, consultation, and regular follow up for high level executive clientele according to EXOS Guidelines.

• Conduct body composition testing, maintain data base, and follow progress of client to ensure they meet and or exceed their goals.

• Document and develop nutritional and supplementation plans for our client.

• Presentation of EXOS Nutrition methodology and philosophy via speaking engagements geared towards the high level executive clientele.

Basic Requirements:

• Possess minimum of 5 years of experience

• Must be a current Registered Dietitian with consulting experience.

• A graduate degree in nutrition, exercise physiology, or public health preferred

• Previous leadership and management experience is preferred
