Skills/Qualifications:

• Experience at least 5 years food industry, as claim, labeling or nutritional manager

• Bachelor's Degree in Nutrition, Food technology or Food Chemistry

• Expert in U.S. food laws and regulations

• Profound knowledge with CODEX Alimentarius and general labeling rules

• Outstanding communication skills with experience dealing with government and legal matters

Title: Nutrition Program Assistant

Overview: Perform community coordination activities to improve the nutritional well-being for people living with HIV/AIDS in a comfortable, supportive environment.

Status: Full Time, 40 hours – Benefit Eligible

Reports to: Program Manager

The mission of the Albany Damien Center is to provide unique services in a comfortable, supportive environment to increase the health and quality of life for people living with and affected by HIV/AIDS.

Minimum Qualifications:

1. AA or BA/ BS in health or human services field;

2. 1-2 year experience working in the field of HIV/AIDS or with other chronic illness;

3. Familiarity with the geographic region being served;

4. Good interpersonal, communication, writing, and computer skills.

5. Cultural and linguistic competence for the target population.

6. Driver's license, ability, and means to travel on a flexible schedule as needed.

Responsibilities:

1. Tracks and documents client program participation;

2. Engages in community coordination activities, make and follow-up on referrals for community services, i.e. other food, nutrition, and financial resources;

3. Documents progress notes that summarize client progress, accomplishments,

4. Attends all AIDS Institute and AlbanyDamienCenter required meetings and trainings.

Salary range: $28,000 to $30,000 depending on experience

3. 2-3 years experience developing and facilitating nutrition health education;

4. Possess an understanding of community level work and the importance of collaborating with other organizations;

5. Good communication, writing, and computer skills;

6. Cultural and linguistic competence for the target population.

7. Driver's license, ability, and means to travel on a flexible schedule as needed.

8. Registered Dietician License required



Responsibilities:

1. Adheres to AIDS Institute requirements for the provision of Nutrition Health Education (NHE);

2. Develops nutrition health education plans that incorporate: goals and objectives, frequency and duration of the sessions, HIV specific health and nutrition topics;

3. Reviews RD assessment and dietary recommendations to determine the food and meal service client receives; and type of health education appropriate to meet needs;

4. Monitors, in conjunction with the Program Manager, the appropriateness of the food and meal service provided to the client;

5. Establishes connections and communicates frequently with Medical Nutrition Programs/Practitioners, and RDs and makes referrals as necessary and appropriate;

6. Completes a NHE and Food and Meal program Screening form which includes the reasons for client participation in the health education session;

7. Maintains client records that document attendance, participation, progress, accomplishments, and additional needs, referrals and goals;

8. Ensures client adherence to NHE and food and meal policies and procedures, follows protocol for food gift card/voucher distribution;

9. Acts as Team Leader with Nutrition Program Assistant to conduct community coordination activities;

10. Communicates and collaborates with AIDS Institute staff and responds timely and appropriately to all requests

11. Attends all AIDS Institute and AlbanyDamienCenter required meetings and trainings.

SalaryRange: mid 30's depending on experience

Health Services Manager, Healthy Living Initiative

Harlem Children's Zone, Inc (HCZ) is a multi-site, multi-program community organization that provides a unique and comprehensive interlocking network of education, social and health services, and recreation to more than 21,000 children and adults in Central Harlem. Its goal is to create a "tipping point" in the neighborhood so that children are surrounded by an enriching environment of college-oriented peers and supportive adults. HCZ is nationally and internationally recognized for its early and progressive intervention programs, and has been called "one of the most ambitious social-policy experiments of our time" by The New York Times.



The Healthy Living Initiative (HLI) is an effort to combat the growing public health problem of childhood obesity. The mission of the Healthy Living Initiative is to encourage HCZ students and their families to incorporate healthy food choices and physical activity into their daily lives.



We are currently seeking a dynamic, energetic Health Services Manager to join our team as part of a comprehensive health, wellness and nutrition program initially focusing on middle and high school aged students and their families. The ideal candidate will have experience developing and facilitating holistic health programs. This Health Services Manager will manage behavioral modification programming with nutritionally high-risk students and their families; supervise and collaborate with the site Health Educators and Health Coordinators to best meet the nutritional and wellness needs for these high-risk students'. Additionally, the Health Services Manager will act as a liaison between HCZ program sites and the medical community. This position reports to the Initiative Lead as part of the central team for the Healthy Living Initiative.



Essential Duties and Responsibilities

• Oversee and implement behavioral modification programming, including program assessment/evaluation of program participants

• Coordinate and work closely with staff at various sites to schedule and implement appropriate services for high-risk students

• Supervise and meet routinely with site Health Educators from HLI sites to provide guidance and to share best practices

• Oversee and manage ongoing trainings related to behavioral modification programming

• Identify and disseminate healthy eating active living nutritional information for Health Educators to utilize at their respective HLI site

• In conjunction with central team, co-lead engagements with parents and staff in support of HLI programming

• Provide agency-wide wellness and health programming for students, parents, and staff

• Provide overall assessments and progress reports of all HLI sites' behavioral modification programming

• Showcase successful implementation of the healthy eating active living programs, both internally and externally

• Provide on-going education/training materials and resources as needed

• Serve as liaison between HCZ programs and medical community

• Perform other duties as assigned



Qualification, Skills and Knowledge Requirements

• Master's Degree in Nursing, Nutrition or other related field is required

• Ideal candidate will be an experienced Nurse Practitioner with experience working with an adolescent population in either a community-based organization, health center or school-based setting; some supervisory experience is preferred

• Must be a self-starter with the ability to analyze, absorb, and utilize new information quickly and efficiently

• Ability to develop and manage relationships with diverse individuals, groups, and environments

• Experience with group counseling and training workshop facilitation

• Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint) and Outlook

• Strong organizational, written and verbal communication skills

• Awareness of and sensitivity to cultural and socioeconomic characteristics of populations served

• Must be able to work flexible hours and perform extensive field work
