Dear Potential Participant,

My name is Shaila Rao and I am a Doctoral student in the Department of  
Health and Behavior Studies at Teachers College, Columbia University  
in New York, New York.  I am doing this survey for my dissertation  
work. The study is to evaluate a Heart Health website developed for  
the South Asian community. To participate in the study, you must be  
(1) a South Asian woman , which means you can trace your heritage to  
South Asia (Bangladesh, Bhutan, India, Nepal, Pakistan, or  Sri Lanka)  
 and (2) be 25 years of age or older, and (3) living in the United  
States.

You are being asked to view the Heart Health web pages on the South  
Asian Health Project website  
(http://www.southasianhealth.org/yourhealth/HH/hearthealthmainpage.aspx).  
These pages have information related to eating and preparing healthier  
South Asian foods, exercise, raising heart healthy kids, and much  
more.  After viewing the pages, you will be asked to fill out a survey  
to tell us how feel about the website as well as your knowledge,  
perceptions, and behaviors related to preventing heart disease. You  
will be able to access the survey on the website. This survey will  
take approximately 25-30 minutes to complete. Your assistance will be  
extremely helpful in improving this site and making it more effective  
in reducing the risk of heart disease within the South Asian community.
After 200 people complete the survey, the computer program will  
automatically send out $100.00 Amazon gift certificates to three  
randomly chosen e-mail accounts without in any way linking your  
identity to the survey results.

February is American Heart Month and an important opportunity for

other South Asian women (friends, sisters, aunties, moms, co-workers),  
please forward this email to them so that they may also learn from the  
South Asian Heart Health pages and participate in this survey.
