Post-Discharge Appointment & Care Transitions Associate Program







The Post-Discharge Appointment and Care Transitions Associate Program at NYU Langone Health provides exceptional individuals with an opportunity to gain insight into patient care, clinical operations, and care coordination, while providing a valuable service to the patients. This is a 1-2 year full - time position and an ideal opportunity for gap-year students seeking to enter medical school.







Responsibilities:



 Facilitate the scheduling of follow-up appointments by :



 Communicating directly with patients and care team to coordinate patient preferences, addressing concerns, and communicating needs to clinical staff.



 Contacting primary care physicians and outside consultants.



 Provide education and real-time training to patients and/or family and clinical staff to ensure engagement.



 Participate in clinical care team work rounds and discharge huddles.



 Assist with process and quality improvement projects assigned by the Leadership teams.











Required Skills:



 Bachelor’s degree and planned completion of pre-medical courses.



 Intention to pursue a medical career and transition out of the program within 1-2 years.



 Excellent interpersonal, organizational , communications, and problem solving skills.



 Knowledge of Microsoft Office.



 Ability to work effectively in a fast-paced environment, prioritize tasks, and respond to urgent issues in a timely manner.







Additional Information:



 1-2 year position, 40 hours/week (full time)
